摘要
目的观察中医挂线联合中药熏洗坐浴治疗复杂性肛瘘的临床疗效。方法选择80例复杂性肛瘘患者为研究对象,随机分为治疗组与观察组,各40例。对照组应用中医挂线法治疗,观察组联合应用中药熏洗治疗。治疗后比较2组的临床疗效、肛门功能评分、创面水肿与分泌物评分情况、肛创面愈合时间、肛门疼痛评分、12个月内肛瘘复发率情况等。结果观察组治疗有效率(95.00%,38/40)显著高于对照组(77.50%,31/40)(P<0.05);治疗后,2组分泌物与创面水肿情况均较治疗前明显减轻(P<0.001),且观察组减轻程度优于对照组(P<0.001);治疗后5 d、10 d,2组肛门疼痛评分均较治疗前显著下降(P<0.001),且观察组降低程度更为显著(P<0.001);观察组创面平均愈合时间、肛门功能评分及复发率均优于对照组(P<0.05)。结论中医挂线联合中药熏洗治疗复杂性肛瘘临床疗效显著,可缩短创面愈合时间,改善创面疼痛、水肿症状,减少创口部位分泌物,改善肛门功能,降低术后复发率。
Objective To observe the clinical efficacy of TCM seton cutting method combined with steaming and douche,sitz bath for complicated anal fistula.Methods Eighty patients with complicated anal fistula,diagnosed and treated in the Third Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from July 2016 to June 2018,were selected as subjects and randomly divided into two groups,40 in each group.The control group was treated with traditional Chinese medicine seton cutting method,and the observation group was treated with traditional Chinese medicine steaming and douche,sitz bath on the basis of the control group.After treatment,clinical efficacy,anal function score,wound edema and secretion score,anal wound healing time,anal pain score,and recurrence rate within 12 months were compared.Results The effective rate of the observation group (95.00%) was significantly higher than that of the control group (77.50%) (P<0.05).After treatment,the secretion and wound edema of the two groups were significantly reduced (P<0.001),and the reduction in the observation group was significantly better (P<0.001).On the 5th and 10th day after treatment,the anal pain scores in both groups were significantly lower than those before treatment (P<0.001),and the observation group was more significant (P<0.001).The average healing time,anal function score and recurrence rate of the observation group were all significantly better (P<0.05).Conclusion Traditional Chinese medicine seton cutting treatment combined with steaming and douche,sitz bath for complicated anal fistula has a significant clinical efficacy by shortening wound healing time,improving wound pain and edema symptoms,reducing secretion at wound site,improving anal function,reducing postoperative recurrence rate.
