摘要
目的探讨关节滑液IL-1β、IL-17及TGF-β1在中老年膝骨性关节炎中的表达及临床意义。方法选取2017年11月-2019年6月本院收治的114例中老年膝骨性关节炎患者作为观察组,另将同期本院收治的单纯性膝关节半月板受损患者50例作为对照组。通过酶联免疫吸附试验对2组患者关节滑液内IL-1β、IL-17及TGF-β1表达水平实施分析,应用Spearman相关性分析法分析关节滑液IL-1β、IL-17、TGF-β1表达和X线K-L分级、膝关节炎疼痛程度的相关性。结果观察组关节滑液IL-1β、IL-17表达水平高于对照组,TGF-β1低于对照组(P <0.05);Ⅲ级、Ⅳ级膝骨性关节炎患者IL-1β、IL-17表达高于Ⅱ级患者,Ⅳ级患者IL-1β、IL-17表达高于Ⅲ级患者(P <0.05);Ⅲ级、Ⅳ级膝骨性关节炎患者膝关节炎疼痛WOMAC评分高于Ⅱ级患者,Ⅳ级患者高于Ⅲ级(P <0.05);经Spearman相关性分析发现,膝骨性关节炎患者关节滑液IL-1β、IL-17表达和X线K-L分级、WOMAC疼痛评分呈正相关,关节滑液TGF-β1表达与X线K-L分级、WOMAC疼痛评分呈负相关(P <0.05)。结论中老年膝骨性关节炎患者的关节滑液IL-1β、IL-17表达异常增高,TGF-β1表达异常降低,监测关节滑液IL-1β、IL-17及TGF-β1能有效评估膝骨性关节炎患者的膝关节疼痛程度及病情进展。
Objective To investigate the expression and clinical significance of synovial fluid IL-1β,IL-17 and TGF-β1 in middle-aged and elderly knee osteoarthritis.Methods A total of 114 elderly patients with skeletal osteoarthritis admitted to our hospital from November 2017 to June 2019 were selected as observation groups.In addition,50 patients with simple knee meniscus injury admitted in our hospital were selected as control group.The levels of IL-1β,IL-17 and TGF-β1 in synovial fluid of the two groups were analyzed by enzyme-linked immunosorbent assay(ELISA).Spearman correlation analysis was used to analyze the correlation between IL-1β,IL-17,TGF-β1 expression,X-ray K-L grade and knee arthritis pain.Results The levels of IL-1β and IL-17 in the synovial fluid of the observation group were higher than those in the control group,and TGF-β1 was lower than that in the control group(P<0.05).The levels of IL-1β and IL-17 in grade Ⅲ and Ⅳ knee osteoarthritis patients were higher than those in grade Ⅱ patients.The levels of IL-1β and IL-17 in grade Ⅳ patients were higher than that in grade Ⅲ patients(P<0.05).The WOMAC scores of knee arthritis patients with grade Ⅲ and Ⅳ knee osteoarthritis were higher than those of grade Ⅱ patients,and grade Ⅳ patients were higher than grade Ⅲ(P<0.05).According to Spearman correlation analysis,the levels of IL-1β and IL-17 in synovial fluid of patients with knee osteoarthritis were positively correlated with X-ray KL grade and WOMAC pain score;TGF-β1 expression and X-ray KL grade of joint synovial fluid,WOMAC Pain scores were negatively correlated(P<0.05).Conclusion The levels of IL-1β and IL-17 in the synovial fluid of patients with skeletal osteoarthritis are abnormally increased,and the level of TGF-β1 is abnormally decreased.Monitoring the synovial fluid IL-1β,IL-17 and TGF-β1 can effectively evaluate the degree of knee pain and progression of patients with knee osteoarthritis.
