流式荧光杂交法检测人乳头状瘤病毒基因分型的疑难结果分析及其对策 被引量:1

Analysis of suspicious results of human papillomavirus genotyping by flow fluorescence hybridization

摘要 目的分析HPV基因分型时出现的疑难结果,并探讨相应的对策。方法采用流式荧光杂交法检测2017年和2018年所有HPV标本,按照HPV疑难标本的复检规则和报告细则进行复检和报告,回顾统计分析标本复查率和复查结果一致性,并分析复查不一致的原因及其结果特征。结果2年内共检测64664份HPV标本,符合疑难标本有2203份(3.41%),2017年和2018年复查率差异有统计学意义(P<0.01)。“标本核酸提取失败”为HPV分析中的主要复查原因,“与病史或历史结果不符”为HPV分析后的主要复查原因。2017年和2018年HPV疑难标本的低内参基因(Globin)比例和复查前后结果一致性差异均无统计学意义(P>0.05)。在疑难标本复查不一致的结果中,最终结果以单一型别HPV阳性为主(66.56%),HPV全阴性占小部分比例(6.49%)。结论HPV疑难标本的分析和复查,可有效保证HPV基因分型的各个质量控制环节,避免错误报告的发出,提高了检验质量。 Objective To analyze the suspicious results of human papillomavirus(HPV)genotyping and explore the countermeasures.Methods All samples submitted for HPV screening from 2017 to 2018 were analyzed by flow fluorescence hybridization.The suspicious samples were redetected and reported by the established rules for HPV genotyping.The redetecting rate of HPV suspicious samples and the consistency of the suspicious results were analyzed retrospectively.The reasons and results of the inconsistency were also analyzed.Results A total of 64664 HPV specimens were detected within the two years,among which 2203 suspicious specimens(3.41%)were identified.The difference in the redetecting rate between 2017 and 2018 was statistically significant(P<0.01)."Nucleic acid extraction failure"was the main reason for redetection during HPV analysis,and"inconsistence with medical history or historical results"was the main reason for the post-analysis redetection.There was no statistical significance on the difference either in the low proportion of globin internal reference gene or in the consistency of results before and after redetection for the suspicious samples between 2017 and 2018(P>0.05).Among the inconsistent results for the suspicious samples,the final results were mainly HPV positive for one type(66.56%),while the total negative results accounted for a small proportion(6.49%).Conclusion The analysis and redetection of the suspicious samples can effectively ensure the various aspects of quality control for HPV genotyping,avoid the issue of error report,and improve the detecting quality.
