摘要
目的观察不同大小脂肪颗粒移植进行面部年轻化及轮廓重塑的疗效,总结技术特点及经验。方法回顾性分析自2018年1月至2019年12月于中国医学科学院北京协和医学院整形外科医院形体雕塑与脂肪移植中心收治的210例行自体脂肪移植面部填充患者的疗效,在低负压(-50 kPa)下,结合面部解剖及其老化特点选择注射层次,深层注射采用大脂肪颗粒,浅层注射采用小脂肪颗粒,真皮内注射采用微脂肪颗粒。术后定期随访,从并发症、患者满意度及第三方医师评价方面评估效果。结果本组共210例患者,脂肪移植总量为12.0~60.0 ml,其中大脂肪颗粒10.0~45.0 ml,小脂肪颗粒2.0~16.0 ml,微脂肪颗粒0.6~4.8 ml。行脂肪移植1次者158例,2次者45例,3次者7例。术后随访3~22个月。55例患者术后出现局部淤青,2周左右逐渐消退。无感染、硬结、脂肪液化坏死、供区凹凸不平等其他并发症。患者满意率为93.3%。第三方医师评价:非常好154例(73.3%),好41例(19.5%),一般15例(7.2%)。结论自体脂肪移植在改善面部老化及重塑面部轮廓方面效果较好。合理选择注射层次及脂肪颗粒是手术的关键。
Objective To observe the efficacy of fat particles with different size in facial rejuvenation and contour remodeling,and summarize the technical characteristics and experience.Methods The medical records of 210 patients who received facial autologous fat grafting in Department of Body Contouring and Fat Grafting Center,Plastic Surgery Hospital of Chinese Academy of Medical Sciences and Peking Union Medical College from January 2018 to December 2019 were reviewed.The negative pressure was-50 kPa.The injection level was selected according to the characteristics of facial anatomy and aging.The macrofat was injected to the deep layer,the microfat was injected to the superficial layer,and the microfat was injected into dermis.Regular follow-up was conducted.The evaluation of postoperative improvements including the incidence of complications,patients’satisfaction and the third independent surgeons.Results The total amount of fat grafts ranged from 12.0 ml to 60.0 ml.The amount of macrofat ranged from 10.0 ml to 45.0 ml,the microfat ranged from 2.0 ml to16.0 ml and the microfat ranged from 0.6 ml to 4.8 ml.One-hundred-fifty-eight patients underwent once fat grafting procedure,45 patients underwent two times and 7 patients underwent three times.All patients were followed-up for 3 to 22 months.The local skin bruise occurred in 55 patients and gradually subsided about 2 weeks.No other complications such as infection,induration,fat liquefaction and necrosis and uneven donor site occurred.The patients’satisfaction rate was 93.3%.The evaluation grade of the third independent surgeons:154 patients(73.3%)were excellent,41 patients(19.5%)were good and 15 patients(7.2%)were general.Conclusion Autologous fat grafting is an effective method to reshape facial contour and improve facial aging.Selecting the injection level and different kinds of fat particles reasonably is the key to our operation.Good clinical efficacy can be obtained.
