糖代谢紊乱合并阻塞性呼吸睡眠暂停综合征的临床特点分析及其危险因素 被引量:1

摘要 目的:为探讨糖代谢紊乱合并阻塞性呼吸睡眠暂停综合征患者的临床特点及其危险因素。方法:随机纳入2018年3月~2019年4月石河子大学医学院第一附属医院新诊断的阻塞性呼吸睡眠暂停综合征患者,根据既往病史及OGTT糖耐量实验检查结果将其分为血糖正常合并阻塞性呼吸睡眠暂停综合征组(病例对照组)、糖代谢紊乱合并阻塞性呼吸睡眠暂停综合征组(D总组),并据血糖结果进一步分为D-1组[糖耐量异常和(或)空腹血糖受损合并阻塞性呼吸睡眠暂停综合征],D-2组(2型糖尿病合并阻塞性呼吸睡眠暂停综合征)与分别比较不同组之间临床资料及生化指标,并通过Logistic回归分析OSAHS与糖代谢紊乱相关关系。结果:①较病例对照组,血糖升高各组的BMI、AHI、LDL、D-D、UA、SBP升高,HDL水平降低,差异有统计学意义(P<0.05);②经过多元素Logistic回归分析,高水平的D-D、AHI、BMI,低水平的HDL是OSAHS患者合并糖代谢紊乱的危险因素。结论:阻塞性呼吸睡眠暂停综合征患者合并有D-D、AHI、BMI水平升高和(或)HDL水平下降,其合并糖代谢紊乱的风险增加。 Objective To explore the clinical characteristics and related risk factors of patients with glycometabolism disorder and obstructive sleep apnea hypopnea syndrome.Method Patients with obstructive sleep apnea hypopnea syndrome newly diagnosed in the First Affiliated Hospital of Shihezi University Medical College from March 2018 to April 2019 were randomly included.According to the past medical history and OGTT glucose tolerance test results,they were divided into two groups:normal blood glucose group with obstructive sleep apnea hypopnea syndrome(case control group),glucose metabolism disorder group with obstructive sleep apnea hypopnea syndrome(D group).According to the results of blood glucose,D-1 group(IFG/IGT with obstructive sleep apnea hypopnea syndrome)and D-2 group(T 2DM with obstructive sleep apnea hypopnea syndrome)were further divided.The relationship between OSAHS and glucose metabolism disorder was analyzed by logistic regression.Results①compared with the case control group,BMI,AHI,LDL,D-D,UA,SBP increased and HDL decreased(P<0.05).②Through multi-element logistic regression analysis,high-level D-D,AHI,BMI and low-level HDL were the risk factors of OSAHS patients with glucose metabolism disorder.Conclusion Patients with obstructive sleep apnea have increased levels of D-D,AHI,BMI and/or decreased levels of HDL,and the risk of glucose metabolism disorder is increased.
