摘要
目的通过观察中药温宫通络饮治疗对损伤型内膜修复、妊娠结局的影响,探讨中药治疗损伤型内膜的临床疗效。方法选取2017年3月~2017年12月我院收治的宫腔粘连因素导致IVF助孕失败患者87例,随机分为对照组39例和观察组48例。对照组予行宫腔镜检查诊断损伤型内膜,宫腔镜下宫腔粘连分解术(TCRA)并上环治疗;观察组在对照组治疗基础上增加中药温宫通络饮治疗,连续2个~3个疗程。观测两组临床疗效,在复查宫腔镜下观察及超声监测子宫内膜改善情况,并随访1年内妊娠结局。结果观察组宫腔粘连改善状况高于对照组,特别是中重度的患者高于对照组,差异有统计学意义(P<0.05);治疗后观察组患者监测子宫内膜厚度优于对照组,差异有统计学意义(P<0.05);治疗1年内对照组39例均行再次移植,21例妊娠,妊娠率53.8%,移植周期数53,周期妊娠率40.4%;观察组48例均行再次移植,29例妊娠,妊娠率60.4%,移植周期数60,周期妊娠率48.3%,两组比较差异均有统计学意义(P<0.05)。结论中药温宫通络饮治疗损伤型内膜,临床疗效确切。
Objective By observing the effect of the traditional Chinese medicine Wengongtongluodecoction on the repair of damaged endometrium and the outcome of pregnancy,the clinical efficacy of traditional Chinese medicine in the treatment of damaged endometrium was explored.Methods 87 cases of patients with IVF failure caused by intrauterine adhesions admitted to our hospital from March 2017 to December 2017 were selected and randomly divided into two groups,the control group(39 cases)were examined by hysteroscopy to diagnose the damaged endometrium and treated withTCRA and superior ring under hysteroscopy,48 cases in the treatment group were treated with Wengongtongluo decoction on the basis of the same control group for 3 consecutive courses.The curative effect of the two groups were observed,and the improvement of endometrium was monitored by hysteroscopy and ultrasound.The pregnancy outcome within 1 year was followed up.Results The total effective rate of the treatment group was higher than that of the control group.Especially in moderate and severe patients,the curative effect of the treatment group was better,and there was significant difference between the two groups.The endometrial thickness of the two groups were monitored.In general,the curative effect of the treatment group was better than that of the control group.39 cases of the control group were re-transplanted within one year,21 cases were pregnant,the pregnancy rate was 53.8%,the number of transplantation cycles was 53,the cycle pregnancy rate was 40.4%;All 48 cases in the observation group werere-transplanted,29 cases were pregnant,the pregnancy rate was 60.4%,the number of transplantation cycles was 60,and the cycle pregnancy rate was 48.3%.Conclusion The clinical efficacy of traditional Chinese medicine Wengongtongluo decoction in the treatment of damaged endometrium was accurate.
