摘要
目的观察老年肺源性心脏病患者血清miR-30a表达情况,分析血清miR-30a mRNA相对表达量与老年肺源性心脏病患者预后的关系。方法对103例老年肺源性心脏病患者随访3年,依据随访结果分为死亡组20例和生存组83例。比较2组一般资料及血清总胆固醇(total cholesterol,TC)、三酰甘油(triacylglycerol,TG)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、脑钠肽(brain natriuretic peptide,BNP)、肌钙蛋白I(troponin I,TnI)水平及miR-30a mRNA相对表达量;采用多因素logistic回归分析老年肺源性心脏病患者死亡的影响因素;绘制ROC曲线,评价血清miR-30a mRNA相对表达量预测老年肺源性心脏病患者死亡的价值。结果死亡组血清BNP[(929.43±96.65)ng/L]、TnI[(1.56±0.42)μg/L]、miR-30a mRNA相对表达量(0.21±0.09)]高于生存组[(762.34±89.32)ng/L、(1.17±0.35)μg/L、0.12±0.03](P<0.05);2组年龄、性别比例、体质量、病程、吸烟及合并高血压、糖尿病比率及血清TC、TG、HDL-C、LDL-C水平比较差异均无统计学意义(P>0.05)。血清BNP(OR=1.121,95%CI:1.041~2.015,P=0.012)、TnI(OR=1.001,95%CI:1.001~1.145,P=0.008)、miR-30a mRNA(OR=1.978,95%CI:1.763~2.231,P=0.006)是老年肺源性心脏病患者死亡的影响因素。血清miR-30a mRNA最佳截断值为0.215时预测老年肺源性心脏病患者死亡的AUC为0.867(95%CI:0.747~0.987,P<0.001),灵敏度为85.0%,特异度为98.8%。结论血清miR-30a过表达是老年肺源性心脏病患者死亡的危险因素,检测血清miR-30a mRNA相对表达量可预测老年肺源性心脏病患者的死亡风险。
Objective To observe the changes of miR-30 a in elderly patients with cor pulmonale and investigate the relationship between miR-30 a mRNA and the prognosis in elderly patients with cor pulmonale.Methods Totally 103 elderly patients with cor pulmonale were followed up for 3 years,and were divided into death group and survival group according to the follow-up results.The general data,the levels of serum total cholesterol(TC),triacylglycerol(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),brain natriuretic peptide(BNP)and troponin I(TnI),and the relative expression of miR-30 a mRNA were compared between two groups.Multivariate logistic regression analysis was used to analyze the influencing factors of death,and ROC was drawn to assess the value of miR-30 a mRNA to the prediction of death in elderly patients with cor pulmonale.Results The levels of serum BNP and TnI and the relative expression of miR-30 a mRNA were higher in death group((929.43±96.65)ng/L,(1.56±0.42)μg/L,0.21±0.09)than those in survival group((762.34±89.32)ng/L,(1.17±0.35)μg/L,0.12±0.03)(P<0.05),while the age,gender ratio,body mass,course of disease,smoking history,hypertension,diabetes,and the levels of serum TC,TG,HDL-C and LDL-C showed no significant differences between two groups(P>0.05).Multivariate logistic regression analysis results showed that the serum BNP(OR=1.121,95%CI:1.041-2.0151,P=0.012),TnI(OR=1.001,95%CI:1.001-1.145,P=0.008)and miR-30 a mRNA(OR=1.978,95%CI:1.763-2.231,P=0.006)were the influencing factors of death in elderly patients with cor pulmonale.ROC analysis showed that when the optimal cut-off value of serum miR-30 a mRNA was 0.215,the AUC for predicting death in elderly patients with cor pulmonale was 0.867(95%CI:0.747-0.987,P<0.001),the sensitivity was 85.0%,and the specificity was 98.8%.Conclusion The overexpression of serum miR-30 a is a risk factor of death in the elderly patients with cor pulmonale,and the detection of serum miR-30 a mRNA can predict the risk of death.
