经皮二氧化碳分压监测在硬质支气管镜治疗中的应用

摘要 目的探讨患者接受硬质支气管镜治疗期间以经皮二氧化碳分压监测用于判断通气方式的正确选择。方法抽选该院2019年1—12月接受硬质支气管镜治疗的患者60例作为研究对象,随机分组,对照组(n=30)手术患者麻醉诱导后选择常频喷射通气,观察组(n=30)手术患者麻醉诱导后选择高频喷射通气,两组患者均接受经皮二氧化碳分压监测,分别记录对比两组患者麻醉后不同时间心率、血压变化情况,对比两组患者通气时长与术后恢复自主呼吸时间差异情况。结果观察组在T3(110.34±2.24)次/min、T4(115.69±2.93)次/min、T5(114.89±2.78)次/min、T6(111.51±2.24)次/min、T7(110.74±3.02)次/min心率变化与对照组比较差异有统计学意义(t=6.790、10.140、11.160、10.830、9.380,P<0.05),观察组在T3(107.34±2.75)mmHg、T4(109.43±2.33)mmHg、T5(109.39±2.16)mmHg、T6(109.22±1.76)mmHg、T7(110.03±1.57)mmHg血压变化与对照组比较差异有统计学意义(t=8.860、9.020、9.330、9.150、10.130,P<0.05),观察组术后恢复自主呼吸时间(5.46±1.33)min与对照组比较差异有统计学意义(t=8.850,P<0.05)。结论患者接受硬质支气管镜治疗期间实施经皮二氧化碳分压监测能够反应心率、血压等变化,以便正确选择通气方式。 Objective To explore the use of percutaneous carbon dioxide partial pressure monitoring to determine the correct choice of ventilation during the treatment of rigid bronchoscopy.Methods Sixty patients who received rigid bronchoscopy in the hospital from January to December 2019 were selected as the research objects.They were randomly divided into two groups.The patients in the control group(n=30)selected regular-frequency jet ventilation after induction of anesthesia,and the patients in the observation group(n=30)received anesthesia induction.After choosing high-frequency jet ventilation,the two groups of patients received percutaneous carbon dioxide partial pressure monitoring.The changes in heart rate and blood pressure of the two groups of patients at different times after anesthesia were recorded and compared,and the difference between the duration of ventilation and the time to resume spontaneous breathing after the two groups was compared.Results In the observation group,T3(110.34±2.24)times/min,T4(115.69±2.93)times/min,T5(114.89±2.78)times/min,T6(111.51±2.24)times/min,T7(110.74±3.02)times/min.The change of heart rate per minute was statistically significantly different from that of the control group,the difference was statistically significant(t=6.790,10.140,11.160,10.830,9.380,P<0.05).The observation group was at T3(107.34±2.75)mmHg,T4(109.43±2.33)mmHg,T5(109.39±2.16)mmHg,T6(109.22±1.76)mmHg,T7(110.03±1.57)mmHg blood pressure changes were statistically significantly different from those of the control group,the difference was statistically significant(t=8.860,9.020,9.330,9.150,10.130,P<0.05),the time to recover spontaneous breathing(5.46±1.33)min after operation in the observation group was statistically significantly different from that in the control group,the difference was statistically significant(t=8.850,P<0.05).Conclusion The percutaneous carbon dioxide partial pressure monitoring during the treatment of rigid bronchoscopy can reflect heart rate,blood pressure changes,etc.in order to cho
