PDCA管理循环对提高医院感染手卫生执行率的效果研究

摘要 目的探讨PDCA管理循环对提高医院感染手卫生执行率的临床效果。方法随机从该院未开展PDCA管理模式前医务人员100名,设为对照组A;和300例住院患者设为对照组B。随机选取开展PDCA管理模式后100名,设为研究组A;以及300例住院患者设为研究组B。观察两组患者的医院感染漏报率与感染率和两组医务人员手卫生合格率、手卫生知识合格率。结果研究组B医院感染发生率为9.33%,显著低于对照组B的15.33%(P<0.05);研究组B漏报率为8.00%显著低于对照组B12.00%(P<0.05)。研究组A在无菌操作前、接触体液后、接触患者前、接触患者后、接触周围环境后5项手卫生依从性及手卫生总依从性均显著高于对照组A(P<0.05)。研究组A每一个科室的手卫生依从性均显著高于对照组A。研究组A每一个专业人员的手卫生依从性均显著高于对照组A。研究组A洗手>30 s比例及七步洗手合格率均显著高于对照组A(P<0.05)。结论将PDCA运用到医院感染预防管理中,有利于提高医务人员手卫生依从性,进而降低医院感染发生率。 Objective To explore the clinical effect of PDCA management cycle on improving the implementation rate of nosocomial infection hand hygiene.Methods 100 medical staff before the PDCA management model in the hospital were randomly selected as control group A;and 300 hospitalized patients as control group B.Randomly select 100 people after carrying out the PDCA management model and set them as study group A;and 300 inpatients as study group B.Observe the underreporting rate and infection rate of nosocomial infections of the two groups of patients,and the passing rate of hand hygiene and hand hygiene knowledge of the two groups of medical staff.Results The incidence of nosocomial infection in the study group was B 9.33%significantly,which was lower than that of the control group B 15.33%(P<0.05);the underreporting rate of the study group B was 8.00%significantly,which was lower than that of the control group B 12.00%(P<0.05).The five hand hygiene compliance and total hand hygiene compliance of the study group A before aseptic operation,after contact with body fluids,before contact with patients,after contact with patients,and after contact with the surrounding environment were significantly higher than those of the control group A(P<0.05).The hand hygiene compliance of each department in the study group A was significantly higher than that in the control group A.The hand hygiene compliance of each professional in the study group A was significantly higher than that of the control group A.The proportion of washing hands longer than 30 s and the passing rate of seven-step hand washing in the study group were significantly higher than those in the control group A(P<0.05).Conclusion The application of PDCA to the prevention and management of nosocomial infection is beneficial to improve the compliance of medical staff in hand hygiene,thereby reducing the incidence of nosocomial infection.
