摘要
目的调查医院2018年1~6月和2019年1~6月Ⅰ类切口手术预防性使用抗菌药物情况及比较合理性。方法采用回顾分析方法,对医院2018年1~6月和2019年1~6月的Ⅰ类切口手术病历进行统计、分析。结果2018年1~6月506例患者中预防性使用抗菌药物201例(39.72%),合理率59.70%,2019年1~6月668例患者中预防性使用抗菌药物292例(38.82%),合理率64.38%。2018年1~6月给药时机合理率83.58%、遴选药物合理率83.08%、疗程合理率93.03%。2019年1~6月给药时机合理率83.58%、遴选药物合理率83.08%、疗程合理率93.03%。2018年1~6月Ⅰ类切口抗菌药物种类有6大种类,15个品种,使用频次203次。2019年1~6月Ⅰ类切口抗菌药物种类有5大种类,11个品种,使用频次292次。2018年1~6月Ⅰ类切口分布在7个科室,单一用药频次504次,联合用药频次2次。2019年1~6月Ⅰ类切口7个科室,单一用药频次668次,联合用药频次0次。结论医院Ⅰ类切口手术预防性使用抗菌药物未能达到卫生部有关规定(<30%),仍有遴选药物不适宜,给药时机不适宜,用药疗程不适宜等情况;与2018年1~6月相比2019年Ⅰ类切口手术预防性抗菌药物合理率有所提高。
Objective To investigate the preventive use status of antibiotics and its rationality in class I incision surgery during 1~6 month in 2018 and during 1~6 month in 2019 in our hospital.Methods The clinical data about the preventive use status of antibiotics in class I incision surgery during 1~6 month in 2018 and during 1~6 month in 2019 in our hospital were retrospectively analyzed.Results The decrease of prophylactic antibiotics could improve the rational rate of selection of drugs,the reasonable opportunity of drug administration and the rational rate of treatment course,moreover,the combination medication was decreased.Conclusion The preventive aplication of antibacterial drugs in class I incision surgery in our hospital fails to meet the relevant regulations of the Ministry of Health(<30%).There are still unsuitable drugs for selection,and the drug administration opportunity and the course of medication are not suitable,however,the rational rate of preventive antibacterial drugs is improved during 1~6 month in 2019,as compared with that during 1~6 month in 2018.
