摘要
目的探讨纤维支气管镜在急诊重症患者急救中的应用效果。方法回顾性分析本院收治的84例急诊重症患者的临床资料,根据不同治疗方案将其分成对照组和观察组,每组42例。对照组采用常规治疗,观察组在常规治疗基础上采用纤维支气管镜治疗。比较两组患者的机械通气时间、咳嗽咯痰消失时间、住院时间、血气指标(pH、SaO2、PaO2、PaCO2)、临床疗效和并发症发生情况。结果观察组的机械通气时间、咳嗽咯痰消失时间、住院时间均短于对照组(P<0.05)。治疗后,两组的pH、SaO2、PaO2均升高,PaCO2均降低,且观察组均优于对照组(P<0.05)。观察组的治疗总有效率为92.86%,高于对照组的76.19%(P<0.05)。观察组的并发症总发生率为14.29%,低于对照组的35.71%(P<0.05)。结论应用纤维支气管镜治疗急诊重症患者能够有效缩短患者的症状改善时间、机械通气时间,改善血气指标,减少并发症,提升治疗效果,促进患者早日康复。
Objective To explore the application effect of fiberoptic bronchoscopy in the emergency treatment of emergency severe patients.Methods The clinical data of 84 emergency severe patients admitted in our hospital were retrospectively analyzed.According to different treatment schemes,the patients were divided into control group and observation group,with 42 cases in each group.The control group was given the routine treatment,and the observation group was treated with fiberoptic bronchoscopy on the basis of routine treatment.The mechanical ventilation time,disappearance time of cough and expectoration,hospitalization time,blood gas indexes(pH,SaO2,PaO2,PaCO2),clinical efficacy and complications of the two groups were compared.Results The mechanical ventilation time,disappearance time of cough and expectoration and hospitalization time of the observation group were shorter than those of the control group(P<0.05).After treatment,pH,SaO2,PaO2 of the two groups increased,PaCO2 decreased,and those in the observation group were better than the control group(P<0.05).The total effective rate of treatment of the observation group was 92.86%,which was higher than 76.19%of the control group(P<0.05).The total incidence of complications in the observation group was 14.29%,which was lower than 35.71%in the control group(P<0.05).Conclusion The fiberoptic bronchoscopy applied in the emergency treatment of emergency severe patients can effectively shorten the improvement time of symptoms and mechanical ventilation time,improve the blood gas indexes,reduce complications,improve the treatment effect,and promote the early recovery of patients.
