网球腿的MRI诊断价值及临床意义 被引量:4

摘要 目的:总结网球腿的MRI影像学特征,探讨MRI对网球腿的诊断价值及临床意义。方法:回顾性分析56例经临床诊断为网球腿患者的MRI资料,其中男47例,女9例;MRI检查主要采用冠状面T1加权成像(T1WI)、冠状面及横轴面脂肪抑制-质子加权成像(FS-PDWI)序列对小腿进行成像,由2名高年资放射科诊断医师分析MRI表现并总结网球腿的影像学特征,探讨MRI对网球腿的诊断价值及临床意义。结果:56例患者,均以腓肠肌内侧头(MGM)损伤为主,其中16例(28.6%)合并腓肠肌外侧头(LGM)和/或比目鱼肌(SM)损伤,6例(10.7%)合并跖肌(PT)损伤。56例患者中Ⅰ级损伤8例(14.3%),Ⅱ级损伤33例(58.9%),Ⅲ级损伤15例(26.8%),所有患者均无胫腓骨骨挫伤或骨折。腓肠肌(GM)与比目鱼肌(SM)间积液或积血48例(85.7%);GM筋膜下积液或积血9例(16.1%);小腿浅筋膜损伤39例(69.6%);GM或SM筋膜撕裂11例(19.6%)。结论:MRI可清晰、全面显示网球腿的损伤部位及程度,MGM内侧筋膜下肌肉及肌肉肌腱连接处损伤是其影像学特征。 Objective:To summarize the characteristics of magnetic resonance imaging(MRI)in tennis legs,and to explore the diagnostic value and clinical significance of MRI in tennis legs.Methods:MRI imaging of 56 patients with tennis legs were retrospective analyzed,including 47 males and 9 females.The lower legs underwent MRI with coronal T1 weighted imaging(T1 WI),coronal and transverse fat suppression-proton weighted imaging(FS-PDWI)sequences.Two senior radiologists analyzed the MRI findings and summarized the imaging characteristics of tennis leg.The diagnostic value and clinical significance of MRI in tennis leg were explored.Results:All 56 patients had injury in medial head of the gastrocnemius muscle(MGM).Sixteen cases(28.6%)were combined with injury of lateral gastrocnemius(LGM)and/or soleus muscle(SM),and 6 cases(10.7%)were with injury of plantar muscle(PT).There were three degrees of soft tissue injury according to MRI findings:degreeⅠin8(14.3%)cases,degreeⅡin 33(58.9%)cases,and degreeⅢin 15(26.8%)cases.All patients did not have bone contusion or fracture in tibia or fibula.Forty-eight cases(85.7%)had fluid or blood accumulation in GM and SM,and 9 cases(16.1%)had fluid or blood accumulation in subfascial GM.Thirty-nine cases(69.6%)had superficial fascia injury.Eleven cases(19.6%)had GM or SM fascia tear.Conclusion:MRI can clearly and comprehensively show the injury location and degree of tennis leg.The imaging features were injuries of MGM subfascial muscle and muscle tendon junction.
