Zoomit-DWI序列和常规DWI序列在肾上腺MR小视野平扫的对比研究

摘要 目的研究肾上腺MR小视野扫描序列Zoomit-DWI和常规DWI的对比,探讨在肾上腺小视野弥散成像技术中二者优劣。方法回顾性选择2017年5月~2019年8月在医院行3.0 T MR肾上腺扫描患者48例。分别同时扫描Zoomit-DWI和常规DWI。在参数相同的情况下,作为定量分析,在Zoomit-DWI和常规DWI上以各自b=50的图像作为基准,通过b值400和800以及ADC图,选取病变及正常组织的ADC值对比评估了肾上腺的表观弥散系数(ADC)。作为定性分析,两名主治医师独立评审包含不同b值加权图像的两个图像序列,再对图像质量进行主观评分及一致性检验,分析两个序列对肾上腺扫描的诊断价值。结果48例患者中,恶性肿瘤12例,良性肿瘤17例,正常肾上腺19例。在定量分析中,Zoomit-DWI对良恶性肿瘤及正常肾上腺的ADC值分别为(1.522±0.218)×10^-3 mm^2/s、(1.124±0.158)×10^-3 mm^2/s、(2.021±0.239)×10^-3 mm^2/s。常规DWI对良恶性肿瘤及正常肾上腺的ADC值分别为(1.519±0.184)×10^-3 mm^2/s、(1.107±0.133)×10^-3 mm^2/s、(2.013±0.220)×10^-3 mm^2/s(P>0.05)。在定性分析中,两位主治医师对Zoomit-DWI组的评分平均值均显著高于常规DWI组(P<0.05)。Kappa检验显示,两位医师的评分具有良好的一致性。结论Zoomit-DWI和常规DWI对于肾上腺弥散扫描的结果没有统计学差异,但Zoomit-DWI在主观评价显示有较高的图像分辨率和组织清晰度,对比常规DWI序列有一定的技术优势。 Objective To study the comparison of the zoomit-DWI sequence and regular DWI sequence of the small field scanning of adrenal MR,and to discuss the advantages and disadvantages of the two sequence in the adrenal small field diffusion imaging technology.Methods A retrospective selection of 48 patients who underwent 3.0T MR adrenal scan in the hospital from May 2017 to August 2019 were selected.Both Zoomit-DWI and regular DWI were scanned at the same time.In the case of the same parameters,as a quantitative analysis,the images with b=50 on the Zoomit-DWI and regular DWI were used as the benchmark.Through the b values of 400 and 800 and ADC chart,the ADC value of the diseased and normal tissues was selected to compare the apparent diffusion coefficient(ADC)of the adrenal gland.As a qualitative analysis,two image sequences containing different b-value weighted images were independently reviewed by two attending physicians,and then the image quality was subjectively scored and undergone consistency check.The diagnostic value of the two sequences for adrenal gland scanning was analyzed.Results Among the 48 patients,12 cases were with malignant tumors,17 cases were with benign tumors,and 19 cases were with normal adrenal glands.In quantitative analysis,the ADC values of Zoomit-DWI for benign and malignant tumors and normal adrenal glands were(1.522±0.218)×10^-3 mm^2/s,(1.124±0.158)×10^-3 mm^2/s,(2.021±0.239)×10^-3 mm^2/s respectively.The ADC values of regular DWI for benign and malignant tumors and normal adrenal glands were(1.519±0.184)×10^-3 mm^2/s,(1.107±0.133)×10^-3 mm^2/s,(2.013±0.220)×10^-3 mm^2/s respectively(P>0.05).In the qualitative analysis,the average scores of the two attending physicians in the Zoomit-DWI group were significantly higher than those in the regular DWI group(P<0.05).The Kappa test showed that the scores given by the two physicians were in good agreement.Conclusion There is no statistical difference between Zoomit-DWI and regular DWI in the results of adrenal diffusion scan.However,
