肺癌患者膈肌电刺激及围术期肺康复训练对术后呼吸运动功能的影响 被引量:6

Effects of diaphragmatic electrical stimulation and perioperative lung rehabilitation training on postoperative respiratory function and motor function in patients with lung cancer

摘要 目的分析膈肌电刺激及围术期肺康复训练对肺癌患者术后呼吸及运动功能恢复情况的影响。方法选取2018年7月至2019年7月本院收治的94例肺癌拟择期行手术治疗的患者为研究对象,以随机数字表法将患者分为研究组45例及对照组49例;对照组采取常规围术期肺康复训练,研究组则在对照组基础上联合膈肌电刺激训练;比较两组患者肺功能、6 min步行试验、心肺运动试验以及呼吸运动功能变化。结果干预前,两组患者肺功能指标比较,差异均无统计学意义(均P>0.05);干预8周后,两组患者FEV1与FVC肺功能指标均较干预前显著改善,且研究组FEV1、FVC指标分别为(2.32±0.31)L、(2.83±0.18)L,均高于对照组(2.17±0.26)L、(2.69±0.15)L,差异均有统计学意义(均P<0.05);干预前后,两组患者FEV1/FVC指标比较,差异均无统计学意义(均P>0.05)。干预前,两组患者运动功能及运动耐力比较,差异均无统计学意义(均P>0.05);干预8周后,研究组6MWT、AT、W max、VO2max分别为(518.72±19.28)m、(10.79±1.52)ml/min、(87.92±6.23)W、(18.93±4.25)ml(/min·kg),均高于对照组(507.54±17.14)m、(9.82±1.41)ml/min、(80.62±5.31)W、(16.91±3.82)ml(/min·kg),差异均有统计学意义(均P<0.05)。结论肺癌患者应用围术期肺康复训练及膈肌电刺激,有助于改善肺功能,促进术后呼吸及运动功能的恢复,为有效的呼吸训练方案。 Objective To analyze the effects of diaphragmatic electrical stimulation and perioperative lung rehabilitation training on postoperative respiratory function and motor function in patients with lung cancer.Methods A total of 94 patients with lung cancer who were scheduled to undergo surgical treatment in our hospital from July 2018 to July 2019 were selected as the research objects.The patients were divided into a study group of 45 patients and a control group of 49 patients.The control group received routine perioperative lung rehabilitation training,while the study group received diaphragmatic electrical stimulation on the basis of the control group.The lung function,6-min walk test(6MWT),cardiopulmonary exercise test,respiratory function and motor function changes were compared between the two groups.Results After 8 weeks of intervention,FEV1 and FVC lung function indexes of the two groups significantly improved compared with those before intervention,FEV1 and FVC in the study group were(2.32±0.31)L,(2.83±0.18)L,which were higher than those in the control group[(2.17±0.26)L,(2.69±0.15)L](both P<0.05).After 8 weeks of intervention,6MWT,AT,W max,VO2max in the study group were(518.72±19.28)m,(10.79±1.52)ml/min,(87.92±6.23)W,(18.93±4.25)ml/(min·kg),which were higher than those in the control group[(507.54±17.14)m,(9.82±1.41)ml/min,(80.62±5.31)W,(16.91±3.82)ml/(min·kg)](all P<0.05).Conclusion The use of perioperative lung rehabilitation training and diaphragmatic electrical stimulation helps to improve lung function of lung cancer patients,promote the recovery of postoperative respiratory and motor functions,which is an effective respiratory training program.
