摘要
目的:探讨腹腔镜辅助经肛门全直肠系膜切除术治疗中低位直肠癌的临床价值。方法:选取2018年5月至2019年3月为5例中低位直肠癌患者行腹腔镜辅助经肛门全直肠系膜切除术的相关临床资料。结果:5例均顺利完成手术,无中转开腹,2例行预防性回肠外置,手术时间平均(302.20±51.72)min,术中出血量(102.35±20.20)mL;术后无死亡、吻合口漏等严重并发症发生,肛门功能正常;术后病理结果均为直肠腺癌。术后随访1年以上,未见肿瘤复发转移。结论:腹腔镜辅助经肛门全直肠系膜切除术治疗中低位直肠癌安全、有效,但需严格把握手术适应证及技术要点。
Objective:To explore the clinical value of laparoscopic-assisted transanal total mesorectal excision(Lap-TaTME)in the treatment of low and middle rectal cancer.Methods:The clinical data of 5 patients who suffered from middle and low rectal cancer and underwent Lap-TaTME from May 2018 to Mar.2019 were analyzed.Results:Five patients were successfully operated without conversion to open operation,2 cases underwent prophylactic ileostomy.The operation time was(302.20±51.72)min.The intraoperative blood loss was(102.35±20.20)mL.There was no death,anastomotic leakage or other serious complications after operation,and the anal function of all patients were normal.The postoperative pathological results were all rectal adenocarcinoma.During the follow-up for more than 1 year,there was no tumor recurrence or metastasis.Conclusions:Lap-TaTME is safe and effective in the treatment of low and middle rectal cancer,but it is necessary to strictly grasp the indications and technical points of the operation.
