摘要
目的探讨计算机体层摄影(CT)增强扫描对中央型肺癌术前分期的诊断价值。方法选取经病理学检查确诊为中央型肺癌的患者92例,术前均给予CT增强扫描,采用Kappa一致性检验分析CT增强扫描结果与术后病理结果的一致性。结果术后病理诊断结果为T1期26例,T2期51例,T3期11例,T4期4例;N0期55例,N1期14例,N2期23例。CT增强扫描术前T分期与术后病理T分期的一致性Kappa值为0.727,P﹤0.05,CT增强扫描T分期与病理T分期一致77例,高于病理T分期8例,低于病理T分期7例,其诊断准确度为83.70%。CT增强扫描术前N分期与术后病理N分期的一致性Kappa值为0.671,P﹤0.05,CT增强扫描N分期与病理N分期一致74例,高于病理N分期9例,低于病理N分期9例,其诊断准确度为80.43%。腺癌、鳞状细胞癌和腺鳞癌病灶的CT最大强化值和CT值达峰时间比较,差异均无统计学意义(P﹥0.05)。结论CT增强扫描在中央型肺癌术前分期中具有较好的诊断价值。
Objective To investigate the diagnostic value of enhanced CT scan in preoperative staging of central lung cancer.Method 92 cases of central lung cancer comfirmed by pathological examination were selected and given enhanced CT scan,and the consistency of enhanced CT results with the postoperative pathological results were compared using Kappa test.Result Postoperative pathological diagnosis of T stage included 26 cases of T1,51 cases of T2,11 cases of T3,and 4 cases of T4;diagnosis of N stage included 55 cases of N0,14 cases of N1 and 23 cases of N2.The Kappa value of consistency between preoperative T staging in enhanced CT scan and postoperative pathology was 0.727(P<0.05).The T stage of enhanced CT scan was consistent with the pathological T stage in 77 cases(83.70%for diagnostic accuracy),with 8 cases higher and 7 cases lower than the pathological T stage.The Kappa value of consistency between preoperative N staging in enhanced CT scan and postoperative pathology was 0.671(P<0.05).The N stage of enhanced CT scan was consistent with the pathological N stage in 74 cases(80.43%for diagnostic accuracy),with 9 cases higher and 9 cases lower than the pathological N stage.There were no significant differences in the maximum enhancement value and time to peak of CT in adenocarcinoma,squamous cell carcinoma and adenosquamous cell carcinoma(P>0.05).Conclusion Enhanced CT scan has a good diagnostic value in the preoperative staging of central lung cancer.
