摘要
目的探讨降钙素原(PCT)和白细胞介素-6(IL-6)在新生儿细菌性肺炎早期诊断的临床价值。方法随机选取我院新生儿科2019年8月~2020年7月收治的64例细菌性肺炎患儿作为观察组,根据严重程度分为轻症肺炎组33例和重症肺炎组31例,另选取同期住院的非感染性疾病患儿64例作为对照组,比较所有患儿血清PCT、IL-6水平,并分析PCT、IL-6早期诊断新生儿肺炎的敏感度、特异度。结果观察组血清PCT、IL-6水平高于对照组,差异有统计学意义(P<0.05);重症肺炎组PCT水平高于轻症肺炎组,差异有统计学意义(P<0.05);重症肺炎组与轻症肺炎组IL-6水平比较,差异无统计学意义(P>0.05);PCT、IL-6早期诊断新生儿肺炎的敏感度分别为81.30%、87.50%,特异度分别为96.90%、82.80%。结论血清PCT水平可用于评估新生儿细菌性肺炎严重程度,而IL-6早期诊断新生儿细菌性肺炎的敏感度较高,两指标在新生儿细菌性肺炎早期诊断方面有重要价值。
Objective To explore the clinical value of procalcitonin(PCT)and interleukin-6(IL-6)in the early diagnosis of neonatal bacterial pneumonia.Methods 64 children with bacterial pneumonia admitted to the Department of Neonatology of our hospital from August 2019 to July 2020 were randomly selected as the observation group.According to their severity,they were divided into 33 cases of mild pneumonia and 31 cases of severe pneumonia.In addition,64 children with non-infectious diseases hospitalized during the same period were selected as the control group to compare the serum PCT and IL-6 levels of all children,and analyze the sensitivity and specificity of PCT and IL-6 in the early diagnosis of neonatal pneumonia.Results The serum PCT and IL-6 levels of the observation group were higher than those of the control group,the difference was statistically significant(P<0.05);the PCT level of the severe pneumonia group was higher than that of the mild pneumonia group,the difference was statistically significant(P<0.05);There was no significant difference in IL-6 levels between the pneumonia group and the mild pneumonia group(P>0.05);the sensitivity of PCT and IL-6 for early diagnosis of neonatal pneumonia were 81.30%and 87.50%,respectively,and the specificities were 96.90%,82.80%.Conclusion Serum PCT levels can be used to assess the severity of neonatal bacterial pneumonia,and IL-6 is more sensitive in the early diagnosis of neonatal bacterial pneumonia,and the two indicators have important values in the early diagnosis of neonatal bacterial pneumonia.
