摘要
目的:了解本社区卫生服务中心中药饮片使用情况,并分析点评对中药处方调配产生的影响及存在的问题,并实施干预措施,以促进临床合理用药,促进医生处方书写的规范性,加强用药的合理性,进而提高医疗质量,保障医疗安全。方法:随机选取本院2019年中药处方1400张进行点评,找出有问题的处方,并针对性分析解决。结果:中药处方点评涉及很多方面,如中药处方的调配、煎熬等。1400张中药处方中,不合理处方56张,主要问题包括配伍禁忌,药味先煎、后下、包煎等无标注,用法用量错误,医生未注明理由及双签名,处方书写不标准,处方诊断和用药不匹配等。结论:中药处方应用点评制度,可将中药处方合理规范化,因此开方诊断、严格控制中药调剂步骤,尽量避免调剂差错,保证中药使用安全。
Objective:To understand the use of Chinese Herbal Pieces in the community health service center,analyze the impact of comments on the deployment of traditional Chinese medicine prescription and the existing problems,and implement intervention measures to promote clinical rational drug use,promote the standardization of doctor's prescription writing,strengthen the rationality of drug use,improve the medical quality and ensure medical safety.Methods:1400 prescriptions of traditional Chinese medicine in our hospital in 2019 were randomly selected for comment,and the prescriptions with problems were found and analyzed and solved.Results:The review of traditional Chinese medicine prescription involved many aspects,such as the deployment and suffering of Chinese medicine prescription.Among the 1400 prescriptions of traditional Chinese medicine,56 prescriptions were unreasonable.The main problems included incompatibility,no label of matters needing attention,wrong usage and dosage,no reason and double signature by doctors,nonstandard prescription writing,mismatching of prescription diagnosis and medication and so on.Conclusion:The application review system of traditional Chinese medicine prescription can standardize the prescription of traditional Chinese medicine reasonably,therefore,prescription diagnosis,strict control of Chinese medicine dispensing steps,as far as possible to avoid dispensing errors,ensure the safety of traditional Chinese medicine.
