摘要
目的促进临床安全、合理使用艾瑞昔布。方法利用某院医院信息系统(HIS),调阅2019年4月门诊应用艾瑞昔布的处方1527张,以《处方管理办法》《医院处方点评管理规范(试行)》《骨关节炎诊治指南》及艾瑞昔布药品说明书等相关疾病临床诊疗指南为依据,从艾瑞昔布的适应证、用法用量、配伍等方面分析用药合理性。结果女性患者明显多于男性(57.69%比42.31%);患者年龄主要为36~65岁,平均(49.86±14.05)岁;骨科开具处方最多(1009张,66.08%),其次为免疫风湿科(221张,14.47%),再次为疼痛科(152张,9.95%);不合理用药处方152张(9.95%),其中给药频次不当72张(47.37%)、无适应证用药65张(42.76%)、重复用药42张(27.63%)、联合用药不适宜36张(23.68%);处方诊断以骨关节炎最多(536张,35.10%);联合使用最多的药物为地奥司明(313张,20.50%)。结论该院门诊对艾瑞昔布的应用总体较合理,但存在一定的不合理用药现象。临床医师应按规范开具处方,调配、核发药师和临床药师应严格对处方进行审核和点评,提高处方质量,确保临床安全、合理用药。
Objective To promote the safe and rational clinical use of imrecoxib.Methods The hospital’s information management system(HIS)was used to review the prescriptions of the application of imrecoxib in the outpatient in April 2019,which totaled 1527 cases.The″Prescription Management Measures″″Hospital Prescription Review Management Regulations(Trial)″,Imrecoxib Drug Specification,and″Guidelines for the Diagnosis and Treatment of Osteoarthritis″were used as the basis for clinical diagnosis and treatment of related diseases;the indications and usages of imrecoxib,the amount of the drug and the compatibility of the drug were investigated to evaluate the rationality of the drug.Results There were significantly more female patients than males(57.69%vs.42.31%);the tients were mainly 36-65 years old,with an average(49.86±14.05)years.Department of Orthopedics had the most prescriptions(1009,66.08%),followed by the Department of Immunology and Rheumatology(221,14.47%),and the Department of Pain Management(152,9.95%).There were 152(9.95%)irrational prescriptions,of which 72(47.37%)were administered improperly,65(42.76%)without indication,42(27.63%)were repeated medications,and 36(23.68%)suitable combination.Osteoarthritis was the most commonly diagnosed by prescription(536,35.10%);the most commonly used drug was diosmin(313,20.50%).Conclusion The application of imrecoxib in the outpatient department of the hospital is generally reasonable,but there are certain irrational medications.Clinicians should formulate prescriptions according to the norms,and pharmacists and clinical pharmacists should strictly review and comment on prescriptions,improve the quality of prescriptions,and ensure clinical safety and rational use of drugs.
