摘要
目的探讨以家庭为中心的剖宫产照护模式的构建及分娩后母婴立即皮肤接触的干预效果。方法选取我院2019年5-7月符合纳入标准的80例择期手术产妇及新生儿作为研究对象,按随机数字表法分为观察组和对照组各40例。对照组实行常规手术照护模式,观察组实行家属陪伴产妇剖宫产分娩及分娩后母婴立即皮肤接触,比较产妇术前焦虑、术中镇静止痛麻醉用药、满意度及新生儿体温恢复情况。结果观察组产妇术前SAS评分显著低于对照组(P<0.001),新生儿出生30 min体温与出生60 min体温均显著高于对照组(P<0.001);产妇满意度高于对照组(P<0.05)。对照组术中追加镇静止痛麻醉药的概率是观察组的6.152倍(P<0.001);主动要求追加镇静止痛麻醉药的概率是观察组的2.660倍(P=0.158)。结论家属陪伴产妇剖宫产分娩及分娩后母婴立即皮肤接触有利于减轻产妇焦虑,减少镇静止痛麻醉药的使用,提高满意度,并促进新生儿体温恢复。
Objective To evaluate the intervention effect of the family-centered care mode-family accompanying and skin-to-skin(STS)contact after cesarean birth.Methods 80 mothers and newborns underwent elective cesarean section between May and July,2019 were randomly divided into control group(n=40)and intervention group(n=40).Family accompanying and STS were implemented in the observation group while routine care implemented in the control group.The differences of maternal anxiety,administration of anxiolytic and analgesic medication,maternal satisfaction and newborns temperature were compared in two groups.Results Maternal anxiety scores was more higher in control group(P<0.001);Both the temperature of the newborns at 30 minutes and 60 minutes after vaginal delivery was much better in control group(P<0.001);Maternal satisfaction was more higher in observation group(P<0.05);The administration of anxiolytic and analgesic medication was 6.152 times more likely while and the rates of demanding initiatively for these anesthetics was 2.660(P=0.158)in control group when compared to the observation group.Conclusion Family accompanying and STS immediately after cesarean birth may promote positive maternal and infant outcomes including decreasing maternal anxiety,reducing the administration of anxiolytic and analgesic medication,improving maternal satisfaction and promoting the temperature of newborns.
