摘要
目的探讨非结核分枝杆菌肺病(NTM)、活动性肺结核(ATB)及耐多药肺结核(MDR-TB)的CT影像学特征以进行鉴别诊断。方法选取2015年8月~2019年5月于我院接受检查及治疗的NTM肺病患者、ATB患者以及MDR-TB患者各98例,回顾性分析三组患者的胸部CT影像学资料并比较其特点。结果与ATB组和MDR-TB组相比,NTM肺病组结节-支气管扩张型显著增多,结节-肿块型及其它型显著减少(P<0.05)。NTM肺病组薄壁空洞、支气管扩张以及小叶中心性结节增多,而厚壁空洞、肺实变、肺不张、肺毁损、肺体积缩小、肺内钙化、肺门纵隔淋巴结钙化、腺泡结节、胸膜增厚、胸腔积液检出率降低,与ATB组和MDR-TB组相比差异均有统计学意义(P<0.05)。结论NTM肺病更多见薄壁空洞、支气管扩张以及小叶中心性结节,ATB、MDR-TB更多见厚壁空洞、肺实变、肺不张、肺毁损、肺体积缩小、肺内钙化、肺门纵隔淋巴结钙化、腺泡结节、胸膜增厚、胸腔积液等。
Objective To investigate the CT imaging characteristics of non-tuberculous mycobacterium pulmonary disease(NTM),active tuberculosis(ATB)and multidrug resistant tuberculosis(MDR-TB)for differential diagnosis.Methods 98 patients with NTM lung disease,98 patients with ATB and 98 patients with MDR-TB who were examined and treated in our hospital from August 2015 to May 2019 were collected.The chest CT imaging data of the three groups of patients were retrospectively analyzed and their characteristics were compared.Results Compared with ATB group and MDR-TB group,nodule-bronchodilation was significantly increased,nodule-mass type and other types were significantly reduced in NTM lung group(P<0.05).The thin-wall hollow,bronchiectasis and lobular centricity nodules increased,and the thick wall hollow,the consolidation of the lung,atelectasis,pulmonary damage,lung volume reduction,the lungs calcification,lung door mediastinal lymph node calcification,nodules,and pleural thickening and pleural effusion acinus detection rate were reduced in NTM lung disease group,compared with ATB and MDR-TB group,differences were statistically significant(P<0.05).Conclusion NTM lung disease is more common with thin-wall hollow,bronchiectasis and centrilobular nodules.ATB and MDR-TB are more common in thick-wall cavity,lung consolidation,atelectasis,lung destruction,lung volume reduction,intra-pulmonary calcification,hilar mediastinal lymph node calcification,acinar nodules,pleural thickening,pleural effusion and so on.
