摘要
目的探讨血清α-L-岩藻糖苷酶(AFU)、甲胎蛋白(AFP)、异常凝血酶原(PIVKA-Ⅱ)联合检测在肝细胞癌(HCC)中的诊断价值。方法选取2018年1月-10月于本院收治的患者,其中HCC组80例,良性肝病组63例及对照组70例,采用贝克曼AU5400全自动生化仪、Dx I800全自动免疫分析仪和雅培ARCHITECT i2000全自动免疫分析仪分别检测患者血清AFU、AFP和PIVKA-Ⅱ的水平。结果 HCC组AFU、AFP和PIVKA-Ⅱ水平明显高于良性肝病组和对照组,差异有统计学意义(P <0.05)。AFU、AFP、PIVKA-Ⅱ和AFU+PIVKA-Ⅱ+AFP诊断HCC的曲线下面积分别为0.775、0.847、0.878和0.909;联合检测其曲线下面积最大,诊断性能最佳,敏感性也最高。结论联合检测AFU、AFP和PIVKA-Ⅱ优于单独检测,对HCC的早期诊断具有重要意义。
Objective To investigate the value and clinical significance of combined detection of serum α-L-fucosidase(AFU),alpha fetoprotein(AFP)and abnormal prothrombin(PIVKA-Ⅱ)in the diagnosis of hepatocellular carcinoma(HCC).Methods Patients admitted to our hospital from January to October 2018 were enrolled,including 80 patients in the HCC group,63 patients in the benign liver disease group,and 70 patients in the control group.AFU,AFP and PIVKA-Ⅱ were detected by Beckman AU5400 automatic biochemical analyzer,DxI800 automatic immunoassay analyzer and Abbott ARCHITECT i2000 automatic immunoassay analyzer.Results The levels of AFU,AFP and PIVKA-Ⅱ in HCC group were significantly higher than those in benign liver disease group and control group,with the differences statistically significant(P<0.05).The area under the curve of AFU,AFP,PIVKA-Ⅱand AFU+PIVKA-Ⅱ+AFP for the diagnosis of HCC were 0.775,0.847,0.878 and 0.909;combined detection obtained the largest area under the curve,the best diagnostic performance and the highest sensitivity.Conclusion Combined detection of AFU,AFP and PIVKA-Ⅱ is superior to separate detection and is of great significance for early diagnosis of HCC.
