摘要
目的观察血清超氧化物歧化酶(SOD)、基质金属蛋白酶9(MMP-9)水平的变化,并探讨二者与脑白质疏松症(LA)病情的关系及诊断效能。方法选取LA患者78例和健康对照者30例,比较两组血清SOD、MMP-9水平。根据Fazekas量表,将LA患者分为轻、中、重三组,比较不同组间血清SOD、MMP-9水平,采用Pearson相关性分析血清SOD、MMP-9水平与LA病变程度的相关性。受试者工作特征曲线(ROC)评价血清SOD、MMP-9对LA的诊断效能。结果LA组血清SOD水平低于对照组,MMP-9水平高于对照组(P均<0.05)。轻、中、重三组间血清SOD、MMP-9水平比较,P均<0.05。血清SOD水平与Fazekas评分呈负相关(r=-0.753,P<0.05),血清MMP-9水平与Fazekas评分呈正相关(r=0.787,P<0.05)。血清SOD水平诊断LA的AUC为0.829,最佳诊断界值为153.5 U/m L,敏感度为70.5%,特异度为93.3%;血清MMP-9水平诊断LA的AUC为0.828,最佳诊断界值为167.5μg/L,敏感度为69.2%,特异度为86.7%。结论LA患者血清SOD水平降低、MMP-9水平升高。血清SOD、MMP-9水平与LA病情有关,且有助于LA的诊断。
Objective To observe the expression changes of serum superoxide dismutase(SOD)and matrix metallopeptidase 9(MMP-9)and to investigate the relationship with the degree of leukoaraiosis(LA)and the diagnosis values of serum SOD and MMP-9 for LA.Methods Seventy-eight LA patients and 30 healthy controls were selected to compare the serum SOD and MMP-9 levels between the two groups.According to Fazekas scale,LA patients were divided them into three groups:light,medium and heavy groups.The serum SOD and MMP-9 levels were compared between different groups.Pearson correlation analysis was used to analyze the correlation between serum SOD and MMP-9 levels and the degree of LA lesions.The diagnostic efficacy of serum SOD and MMP-9 levels for LA was evaluated by receiver operating characteristic curve(ROC).Results The serum SOD level in the LA group was lower than that in the control group and the serum MMP-9 level was higher than that in the control group(both P<0.05).Significant difference was found in the serum SOD and MMP-9 levels among the three groups(both P<0.05).Serum SOD level was negatively correlated with Fazekas score(r=-0.753,P<0.05).Serum MMP-9 level was positively correlated with Fazekas(r=0.787,P<0.05).The AUC of serum SOD level in diagnosis of LA was 0.829 and the best diagnostic cut-off value was 153.5 U/m L.While the sensitivity was 70.5%and the specificity was 93.3%.The AUC of serum MMP-9 level in diagnosis of LA was0.828 and the best diagnostic cut-off value was 167.5μg/L.While the sensitivity was 69.2%and the specificity was86.7%.Conclusion The level of serum SOD decreases and MMP-9 increases in LA patients;the serum levels of SOD and MMP-9 are related to the condition of LA and are helpful to the diagnosis of LA.
