摘要
目的探讨大前庭导水管综合征(large vestibular aqueduct syndrome,LVAS)患者耳蜗死区(cochlear dead regions,CDR)分布情况。方法采用均衡噪声阈值测试(threshold equalizing noise,TEN)检测27例(51耳)LVAS患者的CDR频率分布,探讨LVAS患者CDR的分布特点。结果27例(51耳)LVAS患者中20例(28耳)患者至少有一个频率出现CDR,CDR检出率为54.90%(28/51);LVAS患者各频率段CDR分布:500 Hz 4例(7.84%)、750 Hz 4例(7.84%)、1000 Hz 4例(7.84%)、1500 Hz6例(11.76%)、2000 Hz 10例(19.61%)、3000 Hz 20例(39.22%)、4000 Hz 21例(41.18%);不同听力损失程度的LVAS患者CDR检出率差异有统计学意义(P<0.05),轻度听力损失1例(12.5%)、中度听力损失5例(41.67%)、重度及以上听力损失22例(70.97%)。结论LVAS患者存在CDR,主要分布于高频区域且听力损失程度越重,存在CDR的可能性越大。
OBJECTIVE To investigate the cochlear dead regions in patients with large vestibular aqueduct syndrome(LVAS).METHODS Threshold equalizing noise(TEN)test was used to detect the frequency distribution of cochlear dead regions(CDR)in 27 LVAS patients(51 ears),and to explore the relationship between cochlear dead regions and hearing loss degree of LVAS patients.RESULTS Out of 27 patients with LVAS,20 patients had cochlear dead regions at least in one ear.28 of 51 ears(54.90%)were detected to have cochlear dead regions,with the distribution of dead zones across test frequencies as follows:500 Hz 4 cases(7.84%),750 Hz 4 cases(7.84%),1000 Hz 4 cases(7.84%),1500 Hz 6 cases(11.76%),2000 Hz 10 cases(19.61%),3000 Hz 20 cases(39.22%),4000 Hz 21 cases(41.18%).The detection rate of cochlear dead zone in LVAS patients with different degrees of hearing loss was statistically significant(P<0.05),that was 1 case in mild hearing loss patients(12.5%),5 cases in moderate hearing loss patients(accounting for 41.67%),and 22 cases in severe and above hearing loss patients(accounting for 70.97%).CONCLUSION LVAS patients have cochlear dead zones,which are mainly distributed in high-frequency areas.The greater the degree of hearing loss,the higher the possibility of cochlear dead regions.
