摘要
目的探究持续硬膜外阻滞镇痛在产妇分娩中的镇痛效果及对产妇泌乳、镇痛满意度的影响。方法回顾性分析2019年1月至2019年12月我科室74例要求行硬膜外阻滞镇痛产妇的临床资料,按照第二产程镇痛方式将其分为对照组和观察组,各37例。对照组产妇自规则宫缩后实施镇痛处理,在第二产程暂停镇痛,改用生理盐水;观察组产妇采用硬膜外阻滞镇痛至产程结束。比较两组的镇痛效果。结果观察组产妇会阴部修复时VAS评分低于对照组,产后2 h出血量少于对照组,开始泌乳时间短于对照组,产后3 d母乳喂养次数多于对照组(P<0.05)。观察组的镇痛满意度评分高于对照组(P<0.05);产后1 h,两组的NPY、Cor水平均升高,但观察组低于对照组(P<0.05)。结论持续硬膜外阻滞镇痛能降低产妇应激反应水平,缩短开始泌乳时间,提高产妇镇痛满意度,值得临床应用。
Objective To explore the analgesic effect of continuous epidural block analgesia in puerpera delivery and its influences on lactation of puerpera and analgesia satisfaction.Methods The clinical data of 74 puerperae requiring epidural block analgesia in our department from January 2019 to December 2019 were retrospectively analyzed.According to analgesic methods in the second stage of labor,the puerperae were divided into control group and observation group,with 37 cases in each group.The control group was given analgesic treatment after regular contraction,and the analgesia was suspended in the second stage of labor,and normal saline was used instead;the observation group was given epidural block analgesia to the end of labor.The analgesic effects of the two groups were compared.Results The VAS score of the observation group was lower than that of the control group during perineal repair,the amount of bleeding at 2 h after delivery was less than that of the control group,the time of starting lactation was shorter than that of the control group,and the times of breast-feeding at 3 d after delivery was more than that of the control group(P<0.05).The analgesic satisfaction score of the observation group was higher than that of the control group(P<0.05);at 1 h after delivery,NPY and Cor levels of the two groups increased,but those of the observation group were lower than the control group(P<0.05).Conclusion Continuous epidural block analgesia can reduce the level of maternal stress response,shorten the time of starting lactation time,and improve the satisfaction of maternal analgesia,which is worthy of clinical application.
