摘要
目的探讨经腹壁彩超联合经会阴彩超在胎盘位置诊断中的临床价值。方法78例进行产前胎盘位置测定的前置胎盘孕妇(产后均确诊为前置胎盘),根据检测方式的不同分为观察组与对照组,各39例。对照组孕妇进行单纯经腹壁彩超检查,观察组孕妇进行经腹壁彩超联合经会阴彩超检查。以孕妇产后实际观察到的情况作为前置胎盘诊断的金标准,对两组孕妇的诊断准确率进行对比。结果对照组漏诊4例,误诊5例,诊断准确30例,诊断准确率为76.92%;观察组漏诊1例,误诊1例,诊断准确37例,诊断准确率为94.87%。观察组诊断准确率高于对照组,差异有统计学意义(P<0.05)。结论经腹壁彩超联合经会阴彩超在胎盘位置诊断中有着良好的诊断作用,对于前置胎盘有着较为准确的判断,可以明显提高临床诊断准确率,降低孕妇及胎儿的死亡率,值得在临床中推广。
Objective To discuss the clinical value of transabdom inal color Doppler ultrasound combined with transperineal color Doppler ultrasound in the diagnosis of placenta previa.Methods A total of 78 pregnant women with placenta previa who had prenatal placenta position determination(all were diagnosed with placenta previa after delivery)were divided into observation group and control group,according to different detection methods,with 39 cases in each group.The pregnant women in the control group underwent simple transabdominal color Doppler ultrasound examination,and the pregnant women in the observation group underwent transabdominal color Doppler ultrasound combined with perineal color Doppler ultrasound examination.The actual observation of pregnant women after delivery was used as the gold standard for diagnosis of placenta previa,and the diagnostic accuracy of the two groups was compared.Results In the control group,4 cases were missed,5 cases were misdiagnosed,30 cases were diagnosed accurately,and the diagnosis accuracy rate was 76.92%;in the observation group,1 case was missed,1 case was misdiagnosed,37 cases were diagnosed accurately,and the diagnosis accuracy rate was 94.87%.The diagnostic accuracy of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Transabdom inal color Doppler ultrasound combined with transperineal color Doppler ultrasound has a good diagnostic role in the diagnosis of placenta previa.It has a more accurate judgment for placenta previa,which can significantly improve the accuracy of clinical diagnosis and reduce the mortality of pregnant women and fetuses.It is worthy of clinical promotion.
