摘要
目的探讨快速人源肿瘤异种移植模型(S-PDTX)技术在儿童复杂难治恶性实体肿瘤组织中应用的可能性。方法首都医科大学附属北京儿童医院肿瘤外科和保定病区就诊的实体肿瘤患儿中,纳入10例化疗无效、化疗中进展或复发病例,术中切除病变组织,依据S-PDTX操作流程,对不同的化疗方案进行小鼠体内药物学检测,筛选出最优的化疗方案。结果10例患儿中,肾脏肿瘤5例,软组织肉瘤3例,肝母细胞瘤1例,神经母细胞瘤1例;其中7例为初次手术标本进行S-PDTX建模,3例为复发标本进行建模;10例患儿均建模成功,进行药敏检测,筛选出有差别的化疗方案;4例患儿已结束化疗,2例因化疗中感染死亡,4例仍在治疗中。结论S-PDTX技术以免疫缺陷小鼠作为肿瘤患儿的“替身”,对不同的化疗方案和药物进行药物敏感性检测,筛选出有差别的治疗方案,可以用于临床用药参考和治疗方案调整。
Objective To investigate the possibility of applying super-patient derived tumor xenograft(S-PDTX)in study of childhood refractory and relapse malignant solid tumors.Methods A database of 10 cases was established for the patient with relapsed or progressive after chemotherapy,sampling from removed tumor tissue,and undergoing the pharmacological test to different drugs and regimes in vivo according to the procedure of S-PDTX,then select out the priority treatment,all the patients were enrolled from the of Oncology Department and Baoding ward of Beijing Children′s Hospital affiliated to Capital Medical University.Results 10 cases including 5 cases of renal tumor,3 cases of soft tissue sarcoma,1 case of hepatoblastoma,1 case of neuroblastoma;7 cases were underwent the test in the first surgery,3 cases were tested in the second surgery because of relapse;all of the cases were success modeled and rule out the superior regime.Four cases were end of treatment,2 cases were died of infection during chemotherapy,and 4 cases are still in treatment.Conclusions Take the immunodeficiency mice as a"substitute"for children with tumors,the S-PDTX can be use to tests the sensitivity for different chemotherapy regime and drugs,then figure out the priority treatment.It can be used for clinical drug application and treatment adjustment.
