摘要
脑血管周围间隙(cPVS)是围绕在脑小血管周围,沿穿支动静脉进入脑实质的特殊结构。cPVS将血管与脑实质分开,其外层边界是神经胶质细胞膜,内层边界是血管外层,内含组织间液。在一些情况下,血管周围间隙会显著增大,具有一定生理病理意义。近年来随着影像学技术的发展,在越来越多的正常人、缺血性及出血性卒中患者的大脑中发现了血管周围间隙扩大(ePVS)。ePVS的病理机制、发病率及临床意义仍不完全清楚,是目前研究的热点问题。本综述主要探讨ePVS的影像学特点、病理机制、危险因素及临床意义,为ePVS的预防及治疗提供理论依据。
Cerebral perivascular space(cPVS)is a special structure that surrounds the cerebral small blood vessels and enters the brain parenchyma along the perforating arteries and veins.cPVS separates blood vessels from the brain parenchyma.The outer boundary is the glial cell membrane,and the inner boundary is the outer layer of the blood vessel,which contains interstitial fluid.In some special cases,the space around the blood vessel will increase significantly,which has certain physiological and pathological significance.In recent years,with the development of imaging technology,enlarged perivascular space(ePVS)has been found in the brain of more and more normal people,ischemic and hemorrhagic stroke patients.The pathological mechanism,morbidity and clinical significance of ePVS are still not fully understood,and it is a hot issue in current research.This review mainly discusses the imaging characteristics,pathological mechanisms,risk factors and clinical significance of ePVS,and provides a theoretical basis for the prevention and treatment of ePVS.
