摘要
目的探讨骨质疏松症患者补肾健脾强骨汤联合唑来膦酸治疗前后血Klotho蛋白、激素水平变化及意义。方法选取2018年6月—2019年12月我院骨质疏松症患者156例,按随机数字表法分为联合组(n=52)、唑来膦酸组(n=52)、汤剂组(n=52)。常规治疗基础上,联合组予以补肾健脾强骨汤联合唑来膦酸,唑来膦酸组予以唑来膦酸,汤剂组予以补肾健脾强骨汤,均治疗3个月。测定对比3组疗效与治疗前、治疗3个月后血Klotho蛋白、骨质疏松有关激素[甲状旁腺激素(PTH)、雌二醇(E2)]、骨密度、血清骨代谢指标[抗酒石酸酸性磷酸酶-5b(TRAP-5b)、骨碱性磷酸酶(BALP)]、骨转换指标[Ⅰ型胶原交联C-末端肽(CTX-1)、骨钙素(OC)]水平,以Pearson相关性分析探讨血Klotho蛋白、激素指标水平与骨质疏松症患者骨密度、血清骨代谢、骨转换指标表达的关联性。结果联合组治疗3个月后总有效率高于唑来膦酸组、汤剂组(P<0.05);联合组治疗3个月后血Klotho蛋白、E2水平高于唑来膦酸组、汤剂组,血PTH水平低于唑来膦酸组、汤剂组(P<0.05);联合组治疗3个月后股骨颈、腰椎、桡骨远端1/3处骨密度高于唑来膦酸组、汤剂组(P<0.05);联合组治疗3个月后血清TRAP-5b、BALP水平低于唑来膦酸组、汤剂组(P<0.05);联合组治疗3个月后血清CTX-1水平低于唑来膦酸组、汤剂组,血清OC水平高于唑来膦酸组、汤剂组(P<0.05);Pearson相关性分析可知,血Klotho蛋白、E2水平与骨质疏松患者骨密度、血清OC水平呈正相关,与血清TRAP-5b、BALP、CTX-1水平呈负相关,血PTH水平与骨质疏松患者骨密度、血清OC水平呈负相关,与血清TRAP-5b、BALP、CTX-1水平呈正相关(P<0.05)。结论骨质疏松症患者血Klotho蛋白、骨质疏松相关激素水平与骨密度、骨代谢、骨转换相关,采用补肾健脾强骨汤联合唑来膦酸治疗,可改善血Klotho蛋白、骨质疏松有关激素、骨代谢指标、骨转换�
Objective To investigate the changes and significance of blood Klotho protein and hormone levels in patients with osteoporosis before and after treatment with Bushen Jianpi Qianggu decoction combined with zoledronic acid.Methods 156 patients with osteoporosis in our hospital from June 2018 to December 2019 were selected and divided into the combined group(n=52),zoledronic acid group(n=52),and decoction group(n=52)according to the random number table method.On the basis of conventional treatment,the combined group was given Bushen Jianpi Qianggu decoction combined with zoledronic acid,the zoledronic acid group was given zoledronic acid,and the decoction group was given Bushen Jianpi Qianggu decoction,and three groups were treated for 3 months.The blood Klotho protein and osteoporosis-related hormones including parathyroid hormone(PTH)and estradiol(E2),bone mineral density,serum bone metabolism indexes including anti-tartaric acid phosphatase-5b(TRAP-5b)and bone alkaline phosphatase(BALP),bone turnover indexes including type I collagen cross-linked C-terminal peptide(CTX-1)and osteocalcin(OC)were measured before and after treatment for 3 months.Pearson correlation analysis was used to explore the correlation between blood Klotho protein,hormone index levels and bone mineral density,serum bone metabolism,and bone turnover index expression in patients with osteoporosis.Results The total effective rate of the combined group was higher than that of the zoledronic acid group and the decoction group after 3 months of treatment(P<0.05).The blood Klotho protein and E2 levels were higher in the combined group than the other two groups,but the blood PTH levels were lower in the combined group(P<0.05).The bone density of the femoral neck,lumbar spine,and the distal third of the radius were higher in the combined group than the other two groups(P<0.05).The levels of serum TRAP-5b and BALP were lower in the combined group.The serum CTX-1 levels were lower in the combined group than the other two groups,but the serum OC levels w
