摘要
咳嗽久治不愈者,除从肝、脾、肺、肾治疗外,还可以从心论治。心咳可从"气、血、水"三者之间的关系探讨病机,总的病机为"气虚血瘀,水气上逆,肺失宣肃",病位主要在心、肺、肾三脏。基于咳嗽的病机为本虚标实、虚实夹杂,其治疗原则应标本同治;以心肾为本,以肺为标;以虚为本,以气滞、血瘀、痰饮为标;标实当泻,针对气滞、血瘀、痰饮而梳理气机,活血化瘀,化痰利水;本虚宜补,权衡心脏气、阳之不足,有无兼见肺、肾等脏之亏虚,补气温阳,纠正脏腑之偏衰。
Cough can be treated not only from liver,spleen,lung and kidney,but also from heart.The pathogenesis of heart cough can be discussed from the relationship among"qi,blood and water".The general pathogenesis is qi deficiency and blood stasis,qi and water rushing upward to lung,and impaired dispersing and descending of lung qi.The disease is mainly located in heart,lung and kidney.The principle of treatment should be based on the root cause of heart and kidney and the manifestation of lung,and based on the root cause of deficiency and the manifestation of qi stagnation,blood stasis and phlegm and fluid retention.Excess syndrome should be treated with purgative methods.For qi stagnation,blood stasis and phlegm and fluid retention,qi-flowing should be regulated,the blood circulation should be activated,the phlegm sand fluid retention should be removed.Deficiency syndrome should be treated with tonifying methods.For balancing the deficiency between heart qi and yang,warming yang and benefiting qi,correcting the partial decline of viscera should be considered.
