运用静息态功能MRI评估慢性耳鸣患者突显网络功能 被引量:3

摘要 目的:以突显网络三大重要节点为感兴趣区,运用功能MRI技术探讨慢性主观性耳鸣患者静息态功能连接的改变。方法:共纳入31例耳鸣患者(患者组)及33例年龄、性别和受教育水平相匹配的健康志愿者(对照组)。对每例被试进行神经学量表评估及听力学检测,并使用3.0T磁共振仪采集T1WI结构数据和静息态功能成像数据。提取突显网络的三大节点(左侧前脑岛、右侧前脑岛及背侧前扣带回)作为种子点进行功能连接分析,然后将功能连接出现改变的脑区与耳鸣相关临床特征进行相关性分析。结果:慢性主观性耳鸣患者听力正常,其背侧前扣带回与双侧眶额皮层的功能连接增强,而与右侧中央前回及顶下小叶的功能连接减弱,左侧前脑岛与左侧小脑Ⅶb叶的功能连接增强;背侧前扣带回-顶下小叶的功能连接程度与耳鸣病程(r=-0.538,P=0.002)和耳鸣量表评分(r=-0.559,P=0.001)之间均呈负相关,且背侧前扣带回-中央前回的功能连接程度与耳鸣THQ量表得分呈负相关(r=0.479,P=0.006)。结论:慢性主观性耳鸣患者存在突显网络与多个脑区的功能连接异常,这种改变可能是导致耳鸣的神经生理机制之一。 Objective:To investigate alterations of the functional connectivity using the three nodes in the salience network as seeds in patients with chronic tinnitus.Methods:31 patients with chronic tinnitus and 33 age-,gender-,education level-matched healthy controls were recruited in this study.All subjects underwent audiological tests,neuropsychological tests and MRI scan(T1WI and rest-state functional imaging).Then three nodes from the salience net work were extracted as seeds,which were anchored in bilateral anterior insula and dorsal anterior cingulate.Functional connectivity,as well as correlations between the significant functional connections and clinical characteristics were also conducted.Results:Both the tinnitus group and healthy controls were normal in hearing ability.The dorsal anterior cingulate cortex showed enhanced connectivity with left and right orbito-frontal cortex,but reduced connections with right precentral gyrus and inferior parietal cortex in tinnitus group.And the left anterior insula showed increased connections with left cerebellum lobuleⅦb.Additionally,the connectivity between dorsal anterior cingulate cortex and inferior parietal cortex was negatively correlated with the duration of tinnitus(r=-0.538,P=0.002),and tinnitus questionnaire scores(r=-0.559,P=0.001)in tinnitus subjects.The dorsal anterior cingulate cortex-precentral gyrus connectivity had negative connections with tinnitus questionnaire scores(r=-0.479,P=0.006)in tinnitus group.Conclusion:These findings suggest inefficient modulation of the salience network and abnormal connections between the salience network and other brain areas,this partially attributes to the neurophysiological basis of chronic tinnitus.
