摘要
目的探讨夜班工作对血液透析护士职业倦怠的影响。方法2019年7月24日至8月30日采用基本资料调查问卷及Maslach职业倦怠量表对我国31个省(自治区、直辖市)2738家医院的10570名血透护士进行调查。护士职业倦怠总分=情感衰竭(EE)+去人格化(DP)-个人成就感(PA)+48。根据护士是否承担夜班工作分为夜班组与非夜班组。结果夜班组血透护士职业倦怠总分(40.98±17.58)和EE(17.47±8.84)、DP(4.10±3.73),均显著高于非夜班组(均P<0.001),PA维度得分(28.59±10.05),显著低于非夜班组(P<0.001)。夜班超过0点的血透护士职业倦怠总分(41.23±17.58)、EE(17.57±8.89)、DP(4.16±3.78),均显著高于夜班未超过0点者(均P<0.05)。此外,在承担超过0点夜班的血透护士中,兼顾CRRT工作的血透护士的职业倦怠总分(42.10±17.35)、EE(17.97±8.69)、DP(4.33±3.76),均显著高于单纯血透治疗者(均P<0.05)。结论夜班工作会严重加剧血透护士的职业倦怠程度,超过0点的夜班班次可进一步加重,如同时兼顾CRRT工作,还会进一步加重护血透护士的职业倦怠程度。建议管理者科学进行人力资源调配,平衡夜班班次,尤其是兼顾CRRT工作的血透室,把握好急诊CRRT透析指征,适时给予引导和鼓励等措施有助于改善血透护士的职业倦怠。
Objective To explore the effect of night shift work on job burnout of hemodialysis nurses.Methods A cross-sectional survey on 10570 hemodialysis nurses from 2738 hospitals in 31 provinces was conducted using the sociodemographic questionnaire and Maslach Burnout Inventory between July 24 and August 30,2019.The total burnout score was calculated as emotional exhaustion(EE)+depersonalization(DP)-personal accomplishment(PA)+48.According to whether the nurses undertook night shift work,all the participants were divided into a night shift group and a non-night shift group.Results The total burnout score and scores of EE and DP dimensions were(40.98±17.58),(17.47±8.84),(4.10±3.73)in the night shift group,which were significantly higher than those in the non-night shift group(all P<0.001);the PA dimension score was(28.59±10.05)in the night shift group,which was significantly lower than that in the non-night shift group(P<0.001).The total burnout score and scores of EE and DP dimensions were(41.23±17.58),(17.57±8.89),and(4.16±3.78)in the nurses with night shifts over 12:00 pm,which were significantly higher than those in the nurses with night shifts before 12:00 pm(all P<0.05).The total burnout score and scores of EE and DP dimensions were(42.10±17.35),(17.97±8.69),and(4.33±3.76)in the nurses with night shifts who had a job of continuous renal replacement therapy(CRRT),which were significantly higher than those in the nurses who did not have the CRRT job(all P<0.05).Conclusion Night shift work will seriously aggravate the burnout degree of hemodialysis nurses;and if the shifts exceed 12:00 pm,the degree may be further aggravated.If CRRT work is taken into account,the burnout degree of hemodialysis nurses will be further increased.It is suggested that managers should scientifically allocate human resources and balance night shifts.Especially,if the hemodialysis unit takes CRRT work,managers should grasp the indications of emergency CRRT and give timely guidance and encouragement to alleviate hemodialysis nurses'
