摘要
目的探究用药依从性在不同类型的中青年炎症性肠病患者中的现状及影响因素。方法采用便利抽样法,选取2018年1月至2019年7月在同济大学附属第十人民医院消化内科就诊的154例中青年炎症性肠病患者为研究对象,采用一般资料调查表、Morisky用药依从性问卷、克罗恩病与溃疡性结肠炎知识问卷以及服药信念量表进行问卷调查,通过t检验、χ^2检验及回归分析比较不同类型炎症性肠病患者之间的用药依从性现状以及影响因素。结果克罗恩病患者的CCKNOW得分为7.50 (5.50,9.00)分,服药信念为8.50 (6.00,11.50)分;溃疡性结肠炎的CCKNOW得分为7.50 (6.00,9.50)分,服药信念得分为9.50 (7.00,11.50)分,两个疾病在CCKNOW得分和服药信念未见明显差异(P> 0.05)。线性回归分析显示,疾病类型、年龄、文化程度、疾病活动性、疾病认知以及服药信念是中青年IBD服药依从性的独立影响因素。结论中青年IBD的服药依从性较差,在今后的临床工作中,临床护理人员应重视疾病类型的甄别、年龄及文化程度差异对患者的预警作用,积极提高医护人员的疾病认知和加强对患者的宣教力度,增加患者的服药信念进而提高患者的服药依从性。
Objective To explore the current situation and influencing factors of medication compliance in different types of young and middle-aged patients with inflammatory bowel disease.Methods A total of 154 IBD patients from the Tenth people′s Hospital affiliated to Tongji University to the Department of Gastroenterology from January 2017 to July 2018 were investigated by general data questionnaire,Morisky medication compliance questionnaire,Crohn‘s and Colitis Knowledge Score and Beliefs about Medication Questionnaire.The current situation and influencing factors of patients with different types of inflammatory bowel disease were compared by t-test,chi-square test and regression analysis.Results Crohn′s disease patients had a CKNOW score of 7.50(5.50,9.00)and a medication taking belief of 8.50(6.00,11.50).The CCKNOW score for ulcerative colitis was 7.50(6.00,9.50)and medication belief score was 9.50(7.00,11.50).There was no significant difference in CCKNOW score and medication belief between the two diseases(P>0.05).There was no significant difference in CCKNOW score and medication taking belief between the two diseases(P>0.05),while linear regression analysis was performed.showed that disease type,age,literacy,disease activity,disease perception,and medication taking beliefs are the most important factors in taking medication for inflammatory bowel disease in young and middle-aged adults.Independent influences on compliance.Conclusion In the future clinical work,we should pay attention to the screening of disease type,the early warning effect of age and education level on patients,at the same time,we should actively improve the disease awareness of medical and nursing and strengthen the patient′s propaganda efforts,to increase the patient′s belief in drug taking and thus improve the patient′s drug adherence.
