摘要
目的观察通关利窍针法联合语言训练治疗小儿脑瘫合并语言障碍的临床疗效。方法选取丽水市人民医院康复医学科2016年5月至2019年5月收治的脑瘫合并语言障碍患儿107例,采用单盲法随机分为对照组53例和治疗组54例。对照组制定个体化语言训练方案,治疗组在对照组治疗方法的基础上辅以通关利窍针法,2组均治疗3个月后统计临床疗效,并评估儿童日常生活能力量表(ADL)和粗大运动功能量表(GMFM)评分。结果对照组总有效率为77.36%(41/53),治疗组为92.59%(51/54),2组比较,差异有统计学意义(P<0.05)。2组1~3岁组的总有效率均高于同组4~6岁组,经统计学分析,差异有统计学意义(P<0.05),且治疗组1~3岁的疗效更显著(P<0.05)。2组治疗前GMFM和ADL评分比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组GMFM和ADL评分均明显提高,与同组治疗前比较,差异有统计学意义(P<0.05),且治疗组GMFM和ADL评分提高更显著(P<0.05)。结论通关利窍针法联合语言训练治疗小儿脑瘫合并语言障碍,能明显改善患儿的语言功能、日常生活能力和粗大运动功能,且年龄越小,疗效越好。
Objective To observe the clinical efficacy of Tongguan Liqiao acupuncture therapy combined with speech training in the treatment of children with cerebral palsy combined aphasis.Methods From May 2016 to May 2019,107 children with cerebral palsy combined aphasis admitted into rehabilitation department of Lishui The People’s Hospital were divided into control group with 53 cases and treatment group with 54 cases according to random single blind method.The control group was given individualized speech training program,the treatment group was supplemented with Tongguan Liqiao acupuncture therapy on the basis of the control group.After 3 months of treatment,we kept statistics about the clinical efficacy,and evaluated children’s activities of daily life(ADL) scale and gross motor function measure(GMFM) scale of the 2 groups.Results The total effective rate of the control group was 77.36%(41/53),that of the treatment group was 92.59%(51/54),and the difference was statistically significant between the 2 groups(P<0.05).The total effective rate of the 1-3 age bracket was higher than that of the 4-6 age bracket in the same group, and the difference was statistically significant(P<0.05), moreover, the curative effect of the 1-3 age bracket in the treatment group was more significant(P<0.05).There was no significant difference in GMFM and ADL scores between the 2 groups before treatment(P>0.05),and they were comparable;after treatment, GMFM and ADL scores of the two groups were significantly improved,and the difference was statistically significant compared with the same group before treatment(P<0.05), moreover,the GMFM and ADL scores of the treatment group more significantly improved(P<0.05).Conclusion It can significantly improve children’s language function, ADL and GMFM to use Tongguan Liqiao acupuncture therapy combined with speech training in the treatment of children with cerebral palsy combined aphasis,and the younger the age,the better the curative effect.
