2017年至2019年徐州某三甲医院脑出血患者合并肺部感染的细菌分布及耐药分析 被引量:3

Bacterial distribution and drug resistance of patients with cerebral hemorrhage complicated with pulmonary infection in a third-class hospital in Xuzhou from 2017 to 2019

摘要 目的分析2017年1月至2019年12月徐州某三甲综合医院脑出血患者合并肺部感染的病源菌分布及病源菌耐药情况。方法统计徐州某三甲综合医院2017年1月至2019年12月所有脑出血患者合并肺部感染所送的痰液标本,分离出的阳性细菌进行WHONET统计,分析细菌分布及耐药情况。结果3年内脑出血患者送检的痰液标本共980例,其中分离出的阳性细菌的痰液标本249例,结合临床诊断为肺部感染236例,对236例痰液标本进行细菌分布统计,占前6位的细菌为铜绿假单胞菌、金黄色葡萄球菌、肺炎克雷伯菌、鲍曼不动杆菌、奇异变形杆菌、大肠杆菌。革兰氏阴性杆菌全耐药的药物为氨苄西林、头孢唑啉。革兰氏阳性球菌全耐药的药物为青霉素、红霉素。革兰氏阳性球菌全敏感的药物为万古霉素、利福平、链霉素。革兰氏阴性杆菌全敏感的药物为厄他培南。结论脑出血合并肺部感染患者感染的细菌大部分为革兰氏阴性杆菌,且革兰氏阴性菌耐药情况比革兰氏阳性菌耐药情况严重。 Objective From January 2017 to December 2019,pathogenic bacteria distribution and drug resistance analysis of patients with cerebral hemorrhage complicated with pulmonary infection in a class a general hospital in xuzhou,in order to provide better clinical treatment basis.Methods The sputum samples of all patients with cerebral hemorrhage complicated with pulmonary infection from January 2017 to December 2019 in a third-class general hospital of Xuzhou were collected,and the positive bacteria were collected for WHONET statistics,and the bacterial distribution and drug resistance were analyzed.Results Three years inspection the sputum specimens of 980 cases of cerebral hemorrhage patients,including positive bacteria isolated from the sputum specimens of 249 cases,combined with clinical diagnosis of lung infection in 236 cases,236 cases of sputum specimens for the bacteria distribution statistics,accounts for the first six bacteria for pseudomonas aeruginosa,staphylococcus aureus,klebsiella pneumoniae,acinetobacter baumannii,singular proteus,e.coli.The drugs that were totally resistant to gram-negative bacilli were ampicillin and cefazolin.The drugs of total resistance of gram-positive coccus are penicillin and erythromycin.The drugs of gram-positive coccus were vancomycin,rifampicin and streptomycin.The drug that gram-negative bacilli are all sensitive to is ertapenem.Conclusion It can be seen that most of the infected bacteria are caused by Gram-negative bacilli,the drug resistance of Gram-negative bacteria is more serious than that of Gram-positive bacteria.
