摘要
目的研究内固定联合椎体成形术对脊柱骨折患者椎体高度压缩率及Cobb’s角的影响。方法150例脊柱骨折患者,按治疗方案不同分为对照组与观察组,各75例。对照组应用常规内固定术治疗,观察组应用内固定联合椎体成形术治疗。对比两组患者的手术时间、术中出血量及住院时间,手术前后椎体前缘与后缘高度压缩率、Cobb’s角、伤椎高度,并发症发生情况。结果观察组手术时间(92.13±10.12)min明显长于对照组的(81.26±8.46)min,术中出血量(51.05±4.59)ml少于对照组的(146.92±10.37)ml,住院时间(8.93±1.04)d明显短于对照组的(14.03±2.31)d,差异有统计学意义(P<0.05)。手术后,两组椎体前缘与后缘高度压缩率低于手术前,且观察组显著低于对照组,差异有统计学意义(P<0.05)。手术后,两组患者Cobb’s角小于手术前,伤椎高度高于手术前,差异有统计学意义(P<0.05)。观察组术后并发症发生率1.33%明显低于对照组的10.67%,差异有统计学意义(P<0.05)。结论对于脊柱骨折患者应用内固定联合椎体成形术治疗效果尤为显著,能够有效减轻患者脊髓损伤程度,维持正常的椎体高度,促进患者肢体功能快速康复,值得临床应用。
Objective To study the effect of internal fixation combined with vertebroplasty on the change of vertebral body compression rate and Cobb’s angle in patients with spinal fracture.Methods A total of 150 patients with spinal fracture were divided into control group and observation group according to different treatment regimens,with 75 cases in each group.The control group was treated with conventional internal fixation,and the observation group was treated with internal fixation combined with vertebroplasty.The operation time,intraoperative blood loss,hospitalization time,compression ratio of anterior and posterior vertebral body,Cobb’s angle and the height of the injured vertebrae before and after operation and occurrence of complications were compared between the two groups.Results The operation time(92.13±10.12)min of the observation group was obviously longer than(81.26±8.46)min of the control group,intraoperative blood loss(51.05±4.59)ml was less than(146.92±10.37)ml of the control group,and hospitalization time(8.93±1.04)d was obviously shorter than(14.03±2.31)d of the control group,and the difference was statistically significant(P<0.05).After operation,the compression ratio of anterior and posterior vertebral body of the two groups were lower than those before operation,and the observation group was significantly lower than the control group,and the difference was statistically significant(P<0.05).After operation,the Cobb’s angle of the two groups was smaller than those before operation,and injured vertebrae was higher than those before operation,and the difference was statistically significant(P<0.05).The incidence of postoperative complications 1.33%of the observation group was obviously lower than 10.67%of the control group,and the difference was statistically significant(P<0.05).Conclusion Internal fixation combined with vertebroplasty has particularly significant effect for patients with spinal fractures,and it can effectively reduce the degree of spinal cord injury in patients,maintain no
