摘要
目的:分析Denis B型胸腰椎爆裂骨折患者内固定术后矫正丢失的影响因素.方法:回顾性分析,采集2019年5月至2020年4月嵩县西关骨科医院收治的30例内固定术后发生矫正丢失的Denis B型胸腰椎爆裂骨折患者资料,纳入发生组;另收集同期30例内固定术后未发生矫正丢失的Denis B型胸腰椎爆裂骨折患者资料,纳入未发生组.查阅患者临床资料,将可能的影响因素纳入,经Logistic回归分析找出可能导致Denis B型胸腰椎爆裂骨折患者内固定术后矫正丢失的影响因素.结果:两组内固定方式、功能锻炼情况、骨缺损程度对比,差异有统计学意义(P<0.05);经Logistic回归分析结果显示,4钉固定、功能锻炼不合理、骨缺损程度高是Denis B型胸腰椎爆裂骨折患者内固定术后矫正丢失的危险因素(OR>1,P<0.05).结论:Denis B型胸腰椎爆裂骨折患者内固定术后矫正丢失受4钉固定、功能锻炼不合理、骨缺损程度高的影响.
Objective:To analyze the influencing factors of correction loss after internal fixation in patients with Denis B thoracolumbar burst fracture.Methods:A retrospective analysis was conducted.The data of 30 patients with Denis type B thoracolumbar burst fracture who had correction loss after internal fixation in Songxian Xiguan Hospital Department of Orthopedic from May 2019 to April 2020 were collected as the occurrence group,while the data of 30 patients with Denis B thoracolumbar burst fracture who did not have correction loss after internal fixation during the same period were collected as the non-occurrence group.Reviewed the clinical data of patients,and included possible influencing factors,Logistic regression analysis was used to find out the influencing factors that may lead to the loss of correction after internal fixation in patients with Denis B thoracolumbar burst fracture.Results:Compared methods of internal fixation,functional exercise and the degree of bone defect between the two groups,the difference was statistically significant(P<0.05);By Logistic regression analysis,the results showed that 4-nail fixation,unreasonable functional exercise,and a high degree of bone defect were the risk factors of loss of correction after internal fixation in patients with Denis B thoracolumbar burst fracture(OR>1,P<0.05).Conclusion:The loss of correction after internal fixation in patients with Denis B thoracolumbar burst fracture is affected by 4-nail fixation,unreasonable functional exercise and a high degree of bone defect.
