摘要
目的:分析改进型可调节式上肢抬高康复装置应用于肱骨干骨折患者的短期疗效。方法:选取某院骨三科142例肱骨干骨折患者进行临床对照实验研究。根据护理过程中辅助用具不同分为对照组(使用充气气枕抬高上肢)及试验组(使用改进型可调节式上肢抬高康复装置抬高上肢),每组71例。比较2组患者入院时、术前、术后、术后3 d的患肢肿胀值、舒适状况量表(general comfort questionnaire,GCQ)、视觉模拟评分(visual analogue scale,VAS)、焦虑自评量表(self-rating anxiety scale,SAS)以及E选择素(CD62E)、P选择素(GMP140)、红细胞沉降率(erythrocyte sedimentation rate,ESR)、白细胞介素6(IL-6)等指标和2组患者护理满意情况及并发症发生情况。采用SPSS 20.0进行统计学分析。结果:与入院时比较,2组患者术后3 d的患肢肿胀值减小,VAS、SAS评分明显降低,GCQ评分明显升高,CD62E、GMP140、ESR、IL-6指标明显降低,P均<0.001。术后和术后3 d,试验组患者的患肢膨胀值、VAS评分、SAS评分低于对照组,GCQ评分高于对照组,P均<0.001。术后3 d,试验组患者的CD62E、GMP140、ESR、IL-6指标低于对照组(P均<0.001)。试验组患者的护理满意率高于对照组,并发症发生率低于对照组,P<0.001。结论:改进型可调节式上肢抬高康复装置应用于肱骨干骨折患者围术期临床护理,有助于改善患者术后疼痛程度及焦虑情况,利于患者骨折处肿胀及炎症的控制,减少了并发症,提高了患者满意度。
Objective To analyze the short-term effect of modified adjustable upper limb elevation rehabilitation device on humeral shaft fracture.Methods Totally 142 humeral shaft fracture patients in the department of orthopaedics of some hospital were selected for a controlled clinical trial,who were divided into a control group(using an inflatable air pillow to elevate the upper limb)and an experimental group(using a modified adjustable upper limb elevation rehabilitation device to elevate the upper limb)according to the different aids used in the nursing process,with 71 patients in each group.Swelling values,general comfort questionnaire(GCQ)scores,visual analogue scale(VAS)scores,self-rating anxiety scores(SAS),E-selection(CD62E),P-selection(GMP140),erythrocyte sedimentation rate(ESR),interleukin 6(IL-6),patient satisfaction and complications were compared at admission,preoperatively,postoperatively and 3 days postoperatively in the two groups.Statistical analysis was performed using SPSS 20.0.Results The swelling values of the affected limbs,VAS,SAS and the indexes of CD62E,GMP140,ESR and IL-6 in the two groups of patients were significantly reduced while the GCQ scores were obviously increased preoperatively,postoperatively and at 3 days postoperatively compared with those at the time of admission to the hospital,with all the P values lower than 0.001.The experimental group had the swelling values,VAS and SAS scores lower while GCQ scores higher than those in the control group postoperatively and at 3 days postoperatively,with all the P values lower than 0.001.The CD62E,GMP140,ESR and IL-6 indices of the patients in the experimental group were lower than those in the control group 3 days after surgery(P<0.001).The patients in the experimental group had a higher satisfaction rate while a lower complication rate than those in the control group(P<0.001).Conclusion The device developed can be used for perioperative clinical care of the humeral shaft fracture patient,contributing to relieving postoperative pain and anxiet
