摘要 目的探讨长链非编码RNA富核转录本1(NEAT1)和INK4位点反义非编码RNA(ANRIL)在良恶性胸腔积液鉴别诊断中的临床价值。方法收集良性和恶性胸腔积液患者的胸腔积液标本各50例,采用实时荧光定量聚合酶链反应检测胸腔积液中NEAT1和ANRIL的相对表达量,并依此构建受试者工作特征(ROC)曲线,获取曲线下面积(AUC)并确定临界(cut-off)值,评估NEAT1和ANRIL单独及联合检测对恶性胸腔积液的诊断效能,并与传统肿瘤标志物癌胚抗原(CEA)进行比较。比较不同临床特征恶性胸腔积液患者中NEAT1和ANRIL的相对表达量。结果NEAT1和ANRIL在恶性胸腔积液中的相对表达量均明显高于良性胸腔积液(P﹤0.01)。ROC曲线分析结果显示,NEAT1和ANRIL诊断恶性胸腔积液的AUC值分别为0.815(95%CI:0.732~0.897)和0.807(95%CI:0.723~0.890),均高于CEA诊断恶性胸腔积液的AUC值0.730(95%CI:0.629~0.831)。NEAT1和ANRIL联合检测诊断恶性胸腔积液的灵敏度、特异度、阳性预测值、阴性预测值、准确度均高于NEAT1、ANRIL和CEA单独诊断的结果。不同肿瘤组织来源、肺癌组织类型、性别、年龄、吸烟史、糖尿病或心血管病发生情况的恶性胸腔积液患者中NEAT1和ANRIL的相对表达量比较,差异均无统计学意义(P﹥0.05)。结论NEAT1和ANRIL在恶性胸腔积液中的表达水平均高于良性胸腔积液。NEAT1和ANRIL表达水平的检测对良恶性胸腔积液的鉴别诊断具有一定的临床意义,有可能成为鉴别良恶性胸腔积液的生物标志物,联合检测NEAT1和ANRIL可进一步提高良恶性胸腔积液的诊断效能。 Objective To investigate the clinical value of long non-coding RNA nuclear enriched abundant transcript 1(NEAT1)and antisense noncoding RNA in the INK4 locus(ANRIL)in the differentiation of benign and malignant pleural effusion.Method Benign and malignant pleural effusions were collected from patients with pleural effusion with 50 samples each,and the relative expression levels of NEAT1 and ANRIL in pleural effusions were detected by real-time fluorescent quantitative polymerase chain reaction,and the construction of receiver operating characteristic(ROC)curve,including the area under the curve(AUC)and determination of the cut-off value were based on the forgoing assay.The diagnostic efficacy of NEAT1 and ANRIL alone and in combination for malignant pleural effusion were evaluated,and the value was compared with carcinoembryonic antigen(CEA),a traditional tumor marker.The relative expression levels of NEAT1 and ANRIL in malignant pleural effusion patients with different clinical characteristics were compared.Result The relative expression levels of NEAT1 and ANRIL in malignant pleural effusion were significantly higher than those in benign pleural effusion(P<0.01).ROC curve analysis results showed that the AUC value of NEAT1 and ANRIL in the diagnosis of malignant pleural effusion were 0.815(95%CI:0.732-0.897)and 0.807(95%CI:0.723-0.890),respectively,which were higher than CEA in the diagnosis of malignant pleural effusion whose AUC value was 0.730(95%CI:0.629-0.831).The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of the combined detection of NEAT1 and ANRIL in the diagnosis of malignant pleural effusion were higher than those of NEAT1,ANRIL and CEA alone.There was no significant difference in the relative expression levels of NEAT1 and ANRIL in patients with malignant pleural effusion with varying tumor tissue sources,lung cancer tissue types,gender,age,smoking history,diabetes or cardiovascular disease events(P>0.05).Conclusion The expression levels of NEAT1 and ANRI
