反复肺炎婴幼儿血清miR-155和免疫球蛋白水平的相关性 被引量:3

Correlation between serum MiR-155 and immunoglobulin level in children with recurrent pneumonia

摘要 目的对反复肺炎婴幼儿给予静脉注射免疫球蛋白(IVIG)治疗前后血清微小RNA-155(mi R-155)水平进行检测,研究其在婴幼儿反复肺炎中的作用及与相关免疫球蛋白表达相关性。方法选取2015年9月-2017年9月在本院治疗的反复肺炎婴幼儿患者96例为研究对象,采用IVIG进行辅助治疗,另取同期在本院检查健康无疾病婴幼儿70例为对照组。采用荧光定量PCR法对血清mi R-155表达水平进行测定;采用免疫散射比浊法对血清Ig A、Ig G、Ig M水平进行测定;分析mi R-155与免疫球蛋白(Ig) A、Ig G、Ig M在反复肺炎婴幼儿血清中表达的相关性;分析影响婴幼儿反复肺炎发生的危险因素。结果反复肺炎患儿血清mi R-155和Ig M水平显著高于对照组(P <0.05),Ig A和Ig G水平显著低于对照组(P <0.05)。IVIG辅助治疗后,有效60例,无效36例,有效组血清mi R-155和Ig M水平低于无效组,Ig A和Ig G水平高于无效组,上述差异均有统计学意义(P <0.05)。反复肺炎患儿mi R-155水平与Ig A、Ig G水平呈负相关,与Ig M水平呈正相关(P <0.05)。血清中mi R-155、Ig A和Ig M水平是影响婴幼儿反复肺炎发生的独立危险因素(P <0.05)。结论 mi R-155在反复肺炎婴幼儿血清中高表达,与患儿血清Ig A、Ig G水平呈负相关,与Ig M水平呈正相关,是影响婴幼儿反复肺炎发生的独立危险因素之一,IVIG辅助治疗可上调反复肺炎患儿血清Ig A、Ig G水平,下调mi R-155和Ig M水平,治疗效果显著。 Objective The levels of serum micro RNA-155( mi R-155) in infant with recurrent pneumonia before and after intravenous immunoglobulin( IVIG) treatment are detected,so as to study the role in infantile recurrent pneumonia and its correlation with the expression of related immunoglobulin. Methods A total of 96 infants with recurrent pneumonia,who treated in our hospital from September 2015 to September 2017 were selected as the study subjects,were treated with intravenous immunoglobulin( IVIG) for adjuvant treatment,and 70 healthy infants and young children who examined in our hospital at the same time were selected as the control group. The expression level of mi R-155 in serum was measured by fluorescence quantitative PCR;the levels of serum immunoglobulin( Ig) A,Ig G and Ig M were measured by immunoturbidimetry;the correlations between mi R-155 and Ig A,Ig G and Ig M in serum of recurrent pneumonia infants were analyzed;the risk factors of recurrent pneumonia in infants were analyzed. Results Serum mi R-155 and Ig M levels in children with recurrent pneumonia were significantly higher than those in the control group( P < 0. 05),the levels of Ig A and Ig G were significantly lower than those in the control group( P < 0. 05). After IVIG,60 cases were effective and 36 cases were ineffective,the serum mi R-155 and Ig M levels in the effective group were lower than those in the ineffective group,and the levels of Ig A and Ig G were higher than those in ineffective group,with the difference statistically significant( P < 0. 05). MiR-155 level was negatively correlated with Ig A and Ig G levels and positively correlated with Ig M levels in children with recurrent pneumonia( P < 0. 05). Serum mi R-155,Ig A and Ig M levels were independent risk factors for recurrent pneumonia in infants( P < 0. 05). Conclusion MiR-155 is highly expressed in serum of infants with recurrent pneumonia,which is negatively correlated with serum Ig A and Ig G levels,and positively correlated with Ig M level.
