摘要
目的:探讨经鼻间歇正压通气(NIPPV)在极早早产儿及极低出生体重儿早期呼吸管理中应用的有效性及安全性。方法:选取2018年1月-2019年9月于本院住院的极早早产儿及极低出生体重儿94例,将其按入院时间编号分组,奇数设为NIPPV组(n=45),偶数设为经鼻持续正压通气(NCPAP)组(n=49)。比较两组治疗1周后的机械通气率、肺表面活性物质(PS)使用率,日呼吸暂停频率及程度。比较两组重度支气管肺发育不良(BPD)、无创及有创辅助通气时间、用氧时间及住院时间。结果:NIPPV组机械通气率、PS使用率、呼吸暂停次数及程度均低于NCPAP组(P<0.05)。NIPPV组重度BPD发生率低于NCPAP组,且无创通气时间、有创通气时间、用氧时间及住院时间均短于NCPAP组(P<0.05)。结论:NIPPV作为极早早产儿及极低出生体重儿早期呼吸管理的初始通气模式是安全有效的,可缩短辅助通气时间,降低呼吸暂停频率,值得临床进一步应用研究。
Objective:To investigate the effectiveness and safety of nasal intermittent positive pressure ventilation (NIPPV) in the early respiratory management of extremely premature and very low birth weight infants.Method:A total of 94 extremely premature and very low birth weight infants hospitalized in our hospital from January 2018 to September 2019 were selected.They were numbered and grouped according to their admission time.The odd numbers were set as the NIPPV group (n=45),and the even numbers were set as the NCPAP group (n=49).Mechanical ventilation rate,pulmonary surfactant (PS) utilization rate,daily apnea frequency and degree were compared between two groups after 1 week of treatment.Severe bronchopulmonary dysplasia (BPD),noninvasive and invasive ventilation time,oxygen use time and hospital stay time were compared between two groups.Result:Mechanical ventilation rate,PS utilization rate,daily apnea frequency and degree of the NIPPV group were lower than those of the NCPAP group (P<0.05).The incidence of severe BPD in the NIPPV group was lower than that in the NCPAP group,and noninvasive and invasive ventilation time,oxygen use time and hospital stay time were lower than those in the NCPAP group (P<0.05).Conclusion:NIPPV is safe and effective as an initial ventilation mode for early respiratory management in extremely premature and very low birth weight infants.It can shorten the duration of auxiliary ventilation and reduce the frequency of apnea,which is worthy of further clinical application.
