摘要
中医药治疗癌因性疲乏具有较好的临床疗效,但缺乏以病机为核心的理论指导。脏腑虚损导致的“正虚”,以及痰、湿、瘀、毒为主的“留滞”是中医对癌因性疲乏病因病机的总体概括。本文基于五脏相关和气血津液体系,提出癌因性疲乏的“虚气留滞”病机,对疾病的动态病机演变进行探讨,总结其辨治思路重点在“培元补虚、开郁通滞”,需要在把握肝、脾脏腑辨证的基础上,根据癌因性疲乏病机传变规律拟定具体治则。“虚气留滞”创新病机理论不仅契合癌因性疲乏的病机规律以及证候变化,还可以动态、系统、有效地指导癌因性疲乏的临床实践。本文丰富并发展了中医治疗癌因性疲乏的相关理论。
Traditional Chinese medicine(TCM)has pretty good clinical effect in treating cancer-related fatigue(CRF),but it lacks theoretical guidance centering on pathogenesis.The‘vital qi deficiency’caused by the deficiency of zang-fu viscera and the‘stagnation’mainly caused by phlegm,dampness,blood stasis and poison are the general generalities on the etiology and pathogenesis of CRF in TCM.In this paper,based on the correlation of five viscera and the system of qi,blood and body fluid,the pathogenesis of‘deficiency-qi stagnation’of CRF was proposed,the dynamic pathogenesis’evolution of the disease was discussed.To sum up the idea of differentiation and treatment,the emphasis is on‘nourishing primordial qi and relieving depression and dredging stagnation’,on basis of disease located in the liver and spleen by syndrome differentiation of zang-fu viscera,specific treatment rules should be formulated according to the transformation law of pathogenesis of CRF.The innovative pathogenesis theory of‘deficiency-qi and stagnation’not only fits the pathogenesis law and syndrome change of CRF,but also can guide the clinical practice of CRF dynamically,systematically and effectively.The related theories of TCM treating CRF has been enriched and developed in thia paper.
