摘要
产后抑郁是一种由于女性产后性激素、心理变化等因素造成的精神障碍常见疾病,临床表现为产后患者胃口不佳、失眠、焦虑、悲观等,影响母婴亲密关系,对孩子的发育和早期教育带来不良影响,严重者会产生自杀、伤害婴儿等想法。中医认为本病主要机制是素体血虚,加之产时失血过多,心血不足,心神失养;或因产时感寒,瘀血内停,又气虚血行不畅而停瘀,以致血瘀气逆,扰乱神明;或素体抑郁,产后情志所伤,肝郁化火,上扰神明,又产后失血,肝血不足,肝不藏魂,神魂颠倒所致。分为肝气郁结、气郁化火、心气血亏虚、瘀血内阻证型,并从心肝辨证论治,总结治疗经验,对产后抑郁的治疗具有一定的参考价值。
Postpartum depression is a common mental disorder caused by factors such as postpartum sex hormones and psychological changes.The clinical manifestations include poor appetite,insomnia,anxiety and pessimism,etc.After delivery,affecting the intimate relationship between mother and baby,and adversely affecting the development and early education of children.For severe cases,thoughts like suicide and hurting infants may occur.Traditional Chinese medicine believes that the main mechanism of the disease is blood deficiency in the body plus excessive blood loss during delivery,insufficiency of heart blood and lack of spirit preservation,or because of cold during delivery,blood stasis and Qi deficiency and poor blood flow,resulting in blood stasis and reversed flow of Qi,disturbing the spirit,or physical depression,postpartum emotional damage and liver depression transforming into fire,disturbing the spirit plus postpartum blood loss,liver blood deficiency andspirit disorders.The disease includes liver Qi stagnation,Qi depression transforming into fire,heart Qi and blood deficiency and stagnation of blood stasis syndromes.In this paper,syndrome differentiation and treatmentwas discussed from heart and liver and treatment experience was summarized,which is of certain reference value for the treatment of postpartum depression.
