摘要
目的探讨多层螺旋CT(MSCT)平扫及增强扫描诊断肺部占位病变的价值。方法选取2017年2月—2019年4月就诊于医院并经病理诊断确诊为肺部占位性病变的170例患者,均接受MSCT平扫及增强扫描。比较不同疾病类型MSCT平扫、增强扫描的CT值、强化增值和不同肺癌病理类型病灶CT净增强值,并以病理诊断结果为金标准,分析MSCT平扫及增强扫描诊断肺癌效能。结果不同疾病类型病灶平扫CT值相比,差异无统计学意义(P>0.05),肺癌强化增值高于肺结核球,低于炎性假瘤,增强CT值高于肺结核球、炎性假瘤,且炎性假瘤高于肺结核球,差异有统计学意义(P<0.05);腺癌CT净增强值高于鳞癌、小细胞癌,鳞癌高于小细胞癌,差异有统计学意义(P<0.05);增强扫描诊断肺癌特异度为90.91%(90/99)、敏感度为87.32%(62/71)、准确度为89.41%(152/170),高于平扫诊断的69.70%(69/99)、64.79%(46/71)、67.65%(115/170),差异有统计学意义(P<0.05)。结论MSCT增强扫描可有效鉴别肺部占位病变和肺癌的病理类型,诊断肺癌的敏感度、准确度、特异度较高。
Objective To investigate the value of multislice spiral CT(MSCT)plain scan and enhanced scan in the diagnosis of pulmonary space occupying lesions.Methods From February 2017 to April 2019,170 patients with lung space occupying lesions confirmed by pathological diagnosis were selected and underwent MSCT plain scan and enhanced scan.The CT value and enhancement value of MSCT plain scan and enhanced scan of different disease types were compared,and the CT net enhancement value of lesions with different pathological types of lung cancer was compared.The diagnostic efficacy of MSCT plain scan and enhanced scan was analyzed based on the pathological diagnosis Results As the gold standard.result There was no significant difference in the CT value of different disease types(P>0.05).The enhancement value of lung cancer was higher than that of tuberculoma,lower than that of inflammatory pseudotumor,and the value of enhanced CT was higher than that of tuberculoma and inflammatory pseudotumor,and the difference was statistically significant(P<0.05);the net enhancement value of adenocarcinoma was higher than that of squamous cell carcinoma and small cell carcinoma,and that of squamous cell carcinoma was higher than that of small cell carcinoma.The specificity,sensitivity and accuracy of enhanced scan were 90.91%(90/99),87.32%(62/71)and 89.41%(152/170)respectively,which were higher than those of 69.70%(69/99),64.79%(46/71)and 67.65%(115/170)of plain scan,and the difference was statistically significant(P<0.05).Conclusion Enhanced MSCT scan can effectively differentiate lung space occupying lesions from pathological types of lung cancer,and has high accuracy,high sensitivity and high specificity in the diagnosis of lung cancer.
