经阴道超声血流参数与宫腔粘连患者子宫内膜容受性的相关性分析 被引量:11

摘要 目的探讨经阴道超声血流参数与宫腔粘连患者子宫内膜容受性的相关性。方法经宫腔镜确诊为宫腔粘连的患者87例为病例组,均于排卵日完成经阴道超声检查,记录内膜容积(EV)、内膜血管指数(VI)、血流指数(FI)、血管血流综合指数(VFI),同期妇科检查正常女性30例为对照组,比较不同宫腔粘连程度患者经阴道超声血流参数的差异及其与子宫内膜容受性的关系。结果病例组EV、VI、FI、VFI均低于正常对照组(P<0.05);不同宫腔粘连程度患者超声各参数比较差异有统计学意义(P<0.05),随宫腔粘连程度增加,EV、VI、FI、VFI降低(P<0.05);宫腔粘连患者内膜容受性指标EV与VI、FI、VFI呈正相关(r=0.539、0.479、0.475,P<0.05)。结论宫腔粘连子宫内膜容受性EV与经阴道超声血流参数改变密切相关,随宫腔粘连程度上升,内膜血流灌注减少,容受性降低,通过三维能量多普勒超声可指导宫腔粘连内膜容受性的评估。 Objective To investigate the relationship between transvaginal ultrasound flow parameters and endometrial receptivity in patients with intrauterine adhesions.Methods Eighty-seven patients with intrauterine adhesions confirmed by hysteroscopy were selected,and all of them underwent transwaginal ulrasonography on the ovultion day.The endometrial volume(EV),endometrial vascular index(VI),endometrial flow index(FI)and endomtrial vascular flow index(VFI)were recorded.30 normal women who underwent gynecological examination during the same period were selected as control.The transvaginal ultrasound flow parameters were compared between patients with different degrees of intrauterine adhesions,and their relationship with endometrial recepivity was analyzed.Results EV,VI,FI and VFI were lower in the intrauterine adhesion group than in the normal control group(P<0.05).There were signifiant diferences in ultrasound parameters between patients with different degrees of intrautrine adhesions(P<0.05).With the aggravation of intrauterine adhesions,EV,VI,FI and VFI were decreased(P<0.05).The endometrial receptivity index EV was positively correlated with VI,FI and VFI in patients with intrauterine adhesions(r=0.539,0.479,0.475,P<0.05).Conclusion Endometrial receptivity EV of patients with intrauterine adhesions is closely related to changes of transvaginal ultrasound flow parameters.With the aggravation of intrauterine adhesions,endometrial blood perfusion decreases,and the receptivity decreases.Three-dimensional power Doppler ulrasound can direct the sssment of endometrial receptivity in patients with intrauterine adhesions.
