摘要
目的观察针刺辅助治疗在体外受精-胚胎移植术(IVF-ET)反复种植失败的不孕症患者中的应用效果。方法将216例IVF-ET反复种植失败的不孕患者随机分为对照组和研究组,每组108例。两组患者均给予常规内膜准备方案、促排卵方案及IVF-ET,研究组同时辅助针刺治疗。对比两组患者治疗前后的中医证候评分、性激素水平、子宫内膜血流分布及助孕结局。结果在移植日,两组患者的中医证候总评分均高于治疗前,且研究组的中医证候总评分高于对照组(均P<0.05)。治疗后内膜转化日,两组血清雌二醇和孕酮水平高于治疗前,且研究组雌二醇水平高于对照组(P<0.05);研究组子宫内膜血流分布优于治疗前及对照组(P<0.05)。研究组的人绒毛膜促性腺激素阳性率及临床妊娠率均高于对照组(均P<0.05),但两组胚胎种植率差异无统计学意义(P>0.05)。结论针刺辅助治疗IVF-ET反复种植失败的不孕症患者,可提高患者血清雌二醇水平,改善子宫内膜血流分布以提升容受性,改善妊娠结局。
Objective To observe the application effect of adjuvant acupuncture therapy in infertility patients with repeated implantation failure following in vitro fertilization and embryo transfer(IVF-ET).Methods A total of 216 IVF-ET patients with repeated implantation failure were randomly divided into control group and study group,with 108 cases in each group.Patients in both groups were given conventional endometrial preparation regimen,ovarian hyperstimulation regimen and IVF-ET,and the study group received adjuvant acupuncture therapy additionally.Traditional Chinese medicine(TCM)syndrome scores,sex hormone levels,endometrial blood flow distribution before and after treatment,and pregnancy outcome after assisted reproductive therapy were compared between the two groups.Results On the day of transplantation,the total scores of TCM syndrome in the two groups were higher than those before treatment and the study group had a higher total score of TCM syndrome than the control group(all P<0.05).On the day of endometrial transformation after treatment,the levels of serum estradiol and progesterone in the two groups were higher than those before treatment,and the study group had a higher estradiol level than the control group(P<0.05);the distribution of endometrial blood flow in the study group was better than that before treatment,or in the control group(P<0.05).The study group exhibited a higher positive rate of human chorionic gonadotropin and a higher clinical pregnancy rate than the control group(all P<0.05),whereas there was no statistically significant difference in rate of embryo implantation between the two groups(P>0.05).Conclusion For infertility patients with repeated implantation failure following IVF-EF,adjuvant acupuncture therapy can raise patients′serum estradiol level,improve endometrial blood flow distribution for receptivity elevation,and ameliorate pregnancy outcome.
