摘要
目的分析彩色多普勒超声在诊断乳腺良恶性肿瘤中的应用价值。方法回顾性分析2016年1月至2018年12月在彩色多普勒超声室行彩色多普勒超声检查后经病理确诊的63例乳腺肿瘤患者的临床资料,比较彩色多普勒超声对良恶性肿瘤的检出率和影像学图像表现。结果经病理诊断确诊良性肿瘤49例,恶性肿瘤14例。经彩色多普勒超声检出良性肿瘤47例,其中1例恶性肿瘤误诊为良性,检出率为93.88%,误诊率2.13%;检出恶性肿瘤11例,检出率为78.57%。良性肿瘤多普勒超声图像多为圆形或者椭圆形,有包膜,与周围乳腺组织分界清晰,边缘光滑锐利,内部呈均匀回声,微小钙化极少见,血流不丰富并呈现低速低阻型血流;恶性肿瘤多普勒彩色多普勒超声图像形状不规则,无包膜,与周围乳腺组织分界不明显,边缘多呈毛刺样改变,内部多为不均匀回声,多数见微小钙化,血流丰富并呈现高速高阻型血流。结论彩色多普勒超声具有操作简单、成本低廉、无侵入性操作、可重复性强、安全无痛无创等优点,易被患者所接受,可为乳腺肿瘤的临床诊断及治疗提供参考。
Objective To analyze the value of color Doppler ultrasound in the diagnosis of breast benign and malignant tumors.Methods The clinical data of 63 cases of breast tumors confi rmed by pathology in color Doppler ultrasound room from January 2016 to December 2018 were retrospectively analyzed.The detection rate and image manifestations of benign and malignant breast tumors by color Doppler ultrasound were compared.Results Pathological diagnosis confi rmed 49 cases of benign tumors and 14 cases of malignant tumors.Forty-seven cases of benign tumors were detected by color Doppler ultrasound,of which 1 case was misdiagnosed as benign,the detection rate was 93.88%,the misdiagnosis rate was 2.13%;11 cases were detected as malignant tumors,the detection rate was 78.57%.The Doppler ultrasound images of benign tumors are mostly round or oval,with capsule,clear boundary with surrounding breast tissue,smooth and sharp edge,homogeneous internal echo,micro-calcifi cation was rare,blood fl ow was not rich and presents low-speed and low-resistance blood fl ow;malignant tumors were irregular in shape,no capsule,and the boundary with surrounding breast tissue was not obvious,the edge was mostly present.Burr-like changes,mostly inhomogeneous internal echoes,most microcalcifi cation,rich blood fl ow and high-speed high-resistance blood fl ow.Conclusion Color Doppler ultrasound has many advantages,such as simple operation,low cost,non-invasive operation,strong repeatability,safety,painless and non-invasive.It is easy to be accepted by patients and can provide valuable imaging data for clinical diagnosis and treatment of breast tumors.
