摘要
目的探讨基于肿瘤全域的表观扩散系数(ADC)直方图分析结果与乳腺癌雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体-2(HER2)和增殖细胞核抗原-67(Ki-67)表达的相关性。资料与方法回顾性分析56例乳腺癌共57枚病灶的MRI资料。利用Fire Voxel软件分析扩散加权成像(DWI)图像,获得ADC直方图参数,包括最小值、最大值、平均值、第10、25、50、75、90百分位数、偏度及峰度。比较乳腺癌ER、PR、HER2和Ki-67阳性组与阴性组间的参数差异,评价ADC各直方图参数与ER、PR、HER2、Ki-67表达的相关性。结果 HER2阳性组ADC值(10th、25th、50th)均高于HER2阴性组,Ki-67高表达组ADC值(偏度和峰度)均高于Ki-67低表达组,差异有统计学意义(P<0.05)。ADC值(10th、25th、50th)与HER2表达呈正相关(r=0.286~0.339,P<0.05);ADC值(偏度和峰度)与Ki-67表达呈正相关(r=0.305、0.296,P<0.05)。结论 ADC直方图分析结果与乳腺癌HER2和Ki-67表达具有一定的相关性,提示全肿瘤ADC直方图分析对预测乳腺癌分子生物学指标具有潜在价值。
Purpose To explore the correlation between the apparent diffusion coefficient(ADC)histogram analysis results and estrogen receptor(ER),progesterone receptor(PR),human epidermal growth factor receptor 2(HER2)and Ki-67 expression of breast cancer.Materials and Methods MRI data of 56 cases of breast cancer with 57 lesions were retrospectively analyzed.FireVoxel software was used to analyze diffusion weighted imaging images to obtain ADC histogram parameters,including minimum,maximum,mean,10th,25th,50th,75th,90th,skewness and kurtosis.The parameters of breast cancer ER,PR,HER2 and Ki-67 positive and negative groups were compared,the correlation between the ADC histogram parameters and the expression of ER,PR,HER2 and Ki-67 was evaluated.Results The ADC values(10th,25th,50th)in the HER2 positive group were significantly higher than those in the HER-2 negative group,and the ADC values(skewness and kurtosis)in the Ki-67 high-expression were statistically higher than those in the Ki-67 low expression group(P<0.05).ADC values(10th,25th,50th)were positively correlated with HER2 expression(r=0.286-0.339,P<0.05);ADC values(skewness and kurtosis)were positively correlated with Ki-67 expression(r=0.305 and 0.296,P<0.05).Conclusion ADC histogram analysis results have a certain correlation with HER2 and Ki-67 expression in breast cancer,it is suggested that ADC histogram analysis of whole tumor has potential value for predicting molecular biological indicators of breast cancer.
