摘要
目的分析脂蛋白相关磷脂酶A2(Lp-PLA2)在急性缺血性脑卒中患者中的水平,探讨Lp-PLA2与脑卒中发生的相关性。方法选择头颅计算机断层扫描(CT)或磁共振成像(MRI)检查确诊的急性缺血性脑卒中患者100例,同期年龄性别匹配的健康对照组50例,检测两组研究对象的血清Lp-PLA2、小而密低密度脂蛋白胆固醇(sdLDLC)、超敏C反应蛋白(HCRP)、同型半胱氨酸(Hcy)、三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)等指标的水平,病例组临床神经功能损害程度采用美国国立卫生院神经功能缺损(NIHSS)评分,对两组研究对象的临床指标水平进行比较,分析急性脑卒中发生的风险因素。结果与对照组相比,急性缺血性脑卒中组Lp-PLA2、sdLDL-C及HCRP、胱抑素C(CyS C)、空腹血糖水平明显升高,差异有统计学意义(P<0.05)。根据TOAST分型,不同组别的NIHSS评分有明显的不同,差异具有统计学意义(P<0.05),其中以大动脉粥样硬化组和不明原因组中的NIHSS评分较高,Logistic回归模型分析显示HCRP(OR=1.304,P=0.037)、Lp-PLA2(OR=1.015,P=0.020)是急性脑卒中发生的风险因素。结论Lp-PLA2水平与缺血性脑卒中的发生密切相关,可能是急性缺血性脑卒中的危险因素。
Objective To explore the association between Lp-PLA2and acute ischemic stroke.Methods A total of 100patients with cerebral ischemic stroke which were confirmed by CT or MRI as case group and 50controls who were similar with case groups in age and gender were recruited.Clinical data of patients in the two groups with relevant data,and the correlation between NIHSS score,serum Lp-PLA2,small-dense low-density lipoprotein cholesterol(sdLDL-C),hypersensitive c-reactive protein(HCRP)and homocysteine(Hcy),three acyl glycerin(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C)and cerebral ischemic stroke related factors were counted and analyzed.Results The levels of Lp-PLA2,sdLDL-C,HCRP,cystatin C and fasting glucose in the case group were significantly higher than those of the case controls(P<0.05).According to TOAST classification,NIHSS scores in the LAA group and SUE group were higher(P<0.05).Logistic regression model analysis showed that HCRP(OR=1.304,P=0.037)and Lp-PLA2(OR=1.015,P=0.020)were risk factors for the acute stroke.Conclusion The level of Lp-PLA2was positively correlated with acute ischemic stroke and may be a risk factor for acute ischemic stroke.
