摘要
目的临床分析血清同型半胱氨酸(HCY)、脂蛋白相关性磷脂酶A2(Lp-PLA2)水平在缺血性脑卒中充氧回输血疗效中的应用效果。方法选取2019年12月—2022年12月山东省德州市陵城区人民医院收治的80例缺血性脑卒中患者为试验组,同时选取同期德州市陵城区人民医院收治的80名健康体检者为对照组;再根据美国国立卫生研究所卒中量表(NIHSS)评分,将试验组分为轻度组(<5分)25例、中度组(5~15分)30例、重度组(>15分)25例,进行回顾性研究。比较两组患者血清HCY、Lp-PLA2、LPA水平,同时对患者给予充氧回输血治疗,比较试验组患者治疗前后HCY、Lp-PLA2、LPA水平。结果试验组患者的HCY、Lp-PLA2、LPA水平高于对照组,差异有统计学意义(P<0.05)。轻、中、重度组HCY、Lp-PLA2、LPA水平比较,差异有统计学意义(P<0.05),其中重度组患者的HCY、Lp-PLA2、LPA水平高于轻度组与中度组,中度组HCY、Lp-PLA2、LPA水平高于轻度,差异有统计学意义(P<0.05)。治疗后试验组患者的HCY、Lp-PLA2、LPA水平均低于治疗前,差异有统计学意义(P<0.05)。治疗后试验组的血浆黏度、全血高切黏度、全血低切黏度、红细胞压积及血小板黏附率均较治疗前降低,差异有统计学意义(P<0.05)。结论缺血性脑卒中患者血清HCY、Lp-PLA2水平较高,且在缺血性脑卒中患者充氧回输血过程中监测血清HCY、Lp-PLA2水平,可作为患者病情监测及预后评估的有效指标。
Objective To analyze the clinical effect of serum homocysteine(HCY)and lipoprotein-associated phospholipase A2(Lp-PLA2)levels in the efficacy of oxygenation and blood transfusion after ischemic stroke.Methods A total of 80 patients with ischemic stroke admitted to Lingcheng District People's Hospital from December 2019 to December 2022 were selected as the study subjects,and served as the experimental group.At the same time,80 healthy people were admitted at the same time were selected as the control group.The experimental group was divided into three subgroups according to National Institute of Health stroke scale(NIHSS),that is,mild subgroup(NIHSS<5 points),moderate subgroup(NIHSS 5~15 points)and severe subgroup(NIHSS≥15 points).The research was done retrospectively.The levels of serum HCY,Lp-PLA2 and LPA were compared between the two groups.At the same time,patients with ischemic stroke were treated with oxygenation and blood transfusion,and the levels of HCY,Lp-PLA2 and LPA before and after treatment were compared.Results The levels of HCY,Lp-PLA2 and LPA in the experimental group were significantly higher than those of the corresponding indices(P<0.05).The levels of HCY,Lp-PLA2 and LPA in severe nsubgroup were significantly higher than those of mild and moderate subgroups,and the levels of indicators above moderate subgroup were higher than those of mild subgroup(P<0.05).The levels of HCY,Lp-PLA2 and LPA in the experimental group after treatment were lower than those before treatment,and the difference was statistically significant before and after treatment(P<0.05).The plasma viscosity,whole blood high shear viscosity,whole blood low shear viscosity,hematocrit and platelet adhesion rate of the experimental group after treatment were decreased significantlycompared with those before treatment(P<0.05).Conclusion The levels of serum HCY and Lp-PLA2 are higher in patients with ischemic stroke,and monitoring the levels of serum HCY and Lp-PLA2 during oxygenation and blood transfusion in patients with ischemic
